| Objective This article focus on evaluating the feasibility and safety of radical laparoscopic myomectomy.Compared with previous laparoscopic myomectomy reserving uterus, discussing that on the base of reserving menses,if radical myomectomy could achieve the main goal ulteriorly without increasing the intraoperative and postoperation complications, influencing postoperative normal life, particularly decreasing the recurrence rate of fibroids in premanopause women.Through contrast analysis, discovering the superiority and deficiency of current surgical treatment, adversely approaching the possibility of becoming more effective for the therapy of uterine leiomyomas. Methods Retrospective analysing the clinical data of 67 patients of 40 years old and above diagnosed with numerous uterine leiomyomas undergoing laparoscopic radical myomectomy in First Affiliated Hospital of Sooshow University from 2014-06 to 2015-04 and following up their postoperative recovery and recurrence condition.Compared the results with the control group composed of 67 patients undergoing traditional laparoscopic myomectomy reserving uterus in the end of 2016 at this hospital to investigate the length of stay of inpatients, bleeding volume in surgery, nutritional condition after surgery, especially the reappear of myomas, menses return, complications appearing and the satisfaction degree of sexual life after surgical therapy and so forth.Obtained significant statistical differences of numerous uterine leiomyomas patients’ various indications between two disparate surgical spectrum of surgical therapeutic.Statistical process utilized statistical software SPSS19.0 to dispose.Adopting chi-square test to analyze data, when P<0.05,there is statistical significance. Results The numerous uterine leiomyomas patients undergoing therapeutic procedures described in this text, after operation, the number of recurrence cases within 2 years accumulative totally 6(8.96%).The recurrence rate beyond 2 years remains to be researched by further followed up.There are 43 cases postoperative that menses recovered normally and regularly(64.17%).The cases of menses that not so regular and irregular are 11(16.42%).There are 4 cases after operation amenorrheaed(5.97%).There are 9 patients having menses after surgery, when followed up,they already menopause(13.43%).Compared with control cohort, the bleeding volume during surgery, postoperative infection, length of stay of inpatient and postoperative complications of this surgery have no statistical difference(P<0.05).Conclusions Patient undergoing standardized radical laparoscopic myomectomy the relapse rate within 2 years after surgery is low(8.96%).The recurrence rate beyond 2 years remains to be researched by further followed up.This surgical therapy could satisfy patient’s fundamental claim of reserving uterus and relatively regular menses.Compared with traditional laparoscopic myomectomy, the bleeding volume during surgery, postoperative infection, length of stay of inpatient and postoperative complications of this surgery have no significant statistical difference. Therefore, for this age of women having numerous uterine leiomyomas and needing operatic treatment, this surgery may be the most effective surgical therapeutics. |
