Study On Syndrome Characteristic Of Elderly AECOPD Patients Based On TCN Syndrome Elements Analysis

Objective:Under the circumstances that there is no standard regulations but huge disputes of researches of modern TCM on the AECOPD syndromes as well as clinical syndrome differentiation of gerontal patients,the research in this thesis will explore and analyze the regular pattern of AECOPD syndromes on genotal patients by applying syndrome-element system,collecting their four-diagnostic-methods information.Outcomes generated from this research is expected to be provided as theoretical basis for the future clinical syndrome differentiation and treatment of modern TCM.Methods:In the research,the questionnaire on four-diagnostic-methods in clinic is designed on the basis of cross-section study which is applicable in clinical epidemiology.The subjects are 310 AECOPD gerontal in-patients selected from Emergency Department,Pulmonary Department,Geriatrics Department of Yunnan TCM Hospital during the period between 2016 and 2017.The method adopted in this research is syndrome element differentiation to distinguish elements of disease location and elements of disease nature distribution of gerontal AECOPD,and to make a diagnosis,by first analyzing frequency distribution of the data collected in questionnaires and then referring to factor analysis and cluster analysis.Result:1.The respiratory symptoms of AECOPD gerontal patients in Kunming,Yunnan Province include coughing,aasmus,chest distress,chest pain,suspine difficulties with aasmus,fatigue,dim and sallow complexion,spontaneous sweating,dysphoria and emaciation.As for the tongue fur,greasy tongue fur and yellow tongue fur are the most common symptoms.The pulse conditions are mostly fast pulse,releasing the truth that the largest proportion of the cases are the phlegm heat syndromes.were in the lung,followed by kidney,epidermis,spleen,liver,and heart;the major pathogenic elements were qi deficiency,Yin and Yin deficiency,followed by drinking,water stopped,Yang deficiency,impotence,cold,wet,blood deficiency,blood stasis,qi stagnation,outside wind.2.In addition to syndrome element differentiation,factor analysis and cluster analysis are adopted in this research.The research finds out that element disease location of gerontal AECOPD mainly include lungs,spleen,kidneys and the exterior,and disease nature distribution mainly include phlegm,heat,Qi asthenia,Yang asthenia,Yin asthenia,water retention,cold,fluid-retention and blood stasis.3.Basic syndromes of gerontal AECOPD are phlegm heat syndromes,splenic Qi asthenia syndrome,kidney Qi asthenia syndrome,kidney Yang asthenia syndrome,lung Yin asthenia syndrome,kidney yin asthenia syndrome,cold fluid-retention syndrome and blood stasis syndrome.Conclusion:By adopting syndrome element differentiation,long with applying factor analysis and cluster analysis in the exploration,this research combines TCM clinical treatment experience with outcomes of factor analysis and cluster analysis,revealing traits of gerontal AECOPD syndromes classification from different perspectives.Besides,this thesis provides ideas and methods to future research on TCM syndromes classification and elements combination regulation via combining syndrome element differentiation with factor analysis and cluster analysis.
