Objective:To analyze the clinical features of early cognitive impairment in leukoaraiosis(LA)patients and the changes of fractional anisotropy(FA)in various region of interest of diffusion tensor imaging(DTI).To explore the relationship between White matter changes and cognitive impairment.Methods:A total of 38 chronic ischemic Leukoaraiosis patients and 20 healthy elderly controls choosed in Shan Xi people’s hospital From August 2015 to August 2016,which received Conventional MRI and DTI examination,Using neuropsychological scales to evaluate the different function of cognitive domains.To detect the Fractional anisotropy(FA)in different regions of Interest.and making Correlational analyses.Results:1.Compared with healthy control group,the characteristics of early cognitive impairment in patients with LA pointed out the overall cognitive function 、executive function、the memory、visual-spatial ability、information processing capability decreased greatly(P<0.05).While there was no significant difference of attention(P>0.05).2.there was significant FA decline in bilateral the anterior horn of lateral ventricle、left superior frontal gyrus、left inferior frontal gyrus、bilateral orbital frontal cortex、right temporal lobe deep、right cingulate gyrus、genu of corpus callosum compared to control group(P<0.05).3.The more serious of the LA classification,the lower the value of FA(P<0.05).4.the memory is positively correlated with the brain region of left anterior horn of lateral ventricle、bilateral orbitalfrontal cortex、right temporal lobe deep white、right cingulate gyrus 、genu of corpus callosu cortex,and its negatively correlated with left inferior frontal gyrus(P<0.05),information processing capability is negatively relatived with right anterior horn of lateral ventricle,while negatively correlated with right temporal lobe deep white(P<0.05),the executive function is negatively relatived with area of left orbital frontal cortex、right temporal lobe deep、right cingulate gyrus 、 genu of corpus callosu cortex(P<0.05),and visual-spatial ability is positively relatived with left orbitalfrontal cortex、right temporal lobe deep、genu of corpus callosu cortex(P<0.05).Conclusion:The early appearance of cognitive impairment of LA,the changes of FA in the part of some brain area.The diffusion tensor imaging contributes to the diagnosis of early cognitive impairment of LA patients.
