Objective To investigate the direct economic burden and indirect economic burden of patients with senile dementia who underwent nursing care in Qingdao internal institutions(rest homes,nursing homes and mental health centers),and to analyze the factors that affect the economic burden of disease in patients withsenile dementia.Then provide feasible recommendations for the rational allocation and use of medical and health resources,and to provide the basis for reducing the economic burden of patients with senile dementia.Methods The self-designed questionnaire was used to collect the basic information and disease cost of the patients through the HIS system in the medical institutions.The data processing software EPIDATA3.0 and statistical analysis software SPSS18.0were used to combine the relevant health economics theory and health statistics method,descriptive analysis of the collected data,nonparametric test and binomial logistic regression analysis;interviews with family members and staff in the family and institutions to understand other relevant information of patients with senile dementia to supplement the questionnaire In the absence of information,according to the survey results of patients with senile dementia in the economic burden of disease and analysis of the main factors affecting the economic burden of the disease.Results The direct economic burden of 312 patients in three institutions(rest homes,nursing homes,mental health center)with senile dementia within one month was834738.93 yuan,and the indirect economic burden was 308686.40 yuan.The average direct economic burden of each patient was 2675.45 yuan,and the average indirect economic burden of the 989.38 yuan;Influencing factors,univariate analysis showed that the type of organization,age,marital status,educational level,occupation type,average household income,income level,average daily social activity time,average daily exercise time,severity of disability,availability of hypertension and medical insurance affect the direct economic burden of patients with senile dementia,and the relationship between marital status,family economic status,average daily exercise time,degree of disability,grade of care service,and time of day-to-day loss of family members affect the indirecteconomic burden of patients with senile dementia;The multivariate analysisshowed that the physical health of the patients with senile dementia was significantly higher than that of the institutiontypes,family average monthly income,whether or not with the impact of hypertension,the severity of disability on the direct economic burden are more significant,and the family average monthly income,the severity of disability,the level of care received by the indirect economic burden are more significant.Conclusion and recommendation There are differences in the economic burden of disease in patients with senile dementia.In addition to the basic situation of patients,whether medical insurance,self-care level and nursing service level are the factors that affect the economic burden of patients' diseases.So care for the elderly population,reduce the incidence of senile dementia;adhere to the public welfare as the guide,the classification of different institutions management;establish and improve the nursing care system for patients with senile dementia to reduce the economic burden of disease;strengthen dementia patients treatment and care,and delay the deterioration of the disease.
