Burden Of Disease Due To Senile Demenita In Changsha City And Social Support Strategy

Objectives (1) To describe the disability-adjusted life years (DALY)lost due to senile dementia in Changsha City and to explore the influential factors.(2) To describe the status of family burden due to senile dementia and to explore the influential factors.(3) To explore a reasonable and comprehensive social support system.Methods It was a cross-section study.Diagnosed senile dementia’s caregivers were admited by purposive sampling from Xiangya Hospital, the Second Affiliated Hospital of Central South University, Hunan Provincial People’s Hospital, the Second People’s Hospital of Hunan Province,the First Peoples’s Hospital of Changsha City, the Fourth People’s Hospital of Changsha City,the Eighth Hospital of Changsha City,and other caregivers introduced by them of the patients in community, from January2011to March2012.Under the informed consent,190volunteer caregivers were investigated by Demographic Information Sheet, Family Burden Scale of patient(FBS), and Information Offer Sheet(informations about calculating DALYs).All date was input and analyzed by Excel2007and SPSS16.0. Statistical methods included descriptive statistics, nonparametric test, Spearman rank correlation and multiple linear stepwise regression.Results (1) FBS’s total score of the caregivers was26.69±9.8.(2)97.3%of the caregivers reported family burden.The positive answering rate of each dimension was above50%,except family atmosphere and physical health dimension.The six dimensions of family burden due to Senile Dementia were:the family’s entertainment (63.16%), the impact on family’s routine life (56.84%), psychological health of family members (53.68%), economic burden (52.10%), family atmosphere (40.52%), physical health of family members (34.21%)(3)Multivariate analysis revealed that the related factors of family burden were:severity of dementia((β=0.362), behaviora and psychological symptoms of dementia (BPSD, β=-0.278), health condition of caregivers(β=-0.257),age and pension of the patients(β= -0.213, β=-0.224,), caregiving hours per day(β=0.140), occupation of caregivers (β=0.123).(4)The average burden of senile dementia was10.36DALYs every person, female was15.33DALYs, male was9.93DALYs.The DALYs of female were higher than that of male. Patients with65-69age group brought largest loss of DALYs to the society.Conclusions (1) The family burden due to senile dementia is common and multidimensional.(2)The most serious family burden due to senile dementia is family’s entertainment.Others descending order are family’s routine life, psychological health of family members and economic burden,relatively less serious impacts are family atmosphere,and physical health of family members.(3)The main influencing factors of family burden are as follows: patient’s age,pension, severity, behaviora and psychological symptoms of dementia, and caregiver’s health condition, occupation, caregiving hours per day.(4) We should increase the social support for those patients who are in more serious condition, have behavior and psychological symptoms of dementia,and lower pension,as well as for those caregivers who are in worse health condition,have lower income,and more caregiving hours per day.
