Investigation Into The Current Situation Of The Doctor-patient Relationship In Psychiatric Hospitals In Hebei Province And Analysis Of Its Influencing Factors

Objective:To develop a questionnaire for the relationship between doctors and patients which is applied to investigate the current situation and influencing factors of doctor-patient relationship in Hebei psychiatric hospital.To provide theoretical and practical basis for the clinical study of the factors affecting the doctor-patient relationship,improve the doctor-patient relationship and reduce the occurrence of medical disputes.Methods:1.The development of the questionnaire on the relationship between doctors and patients in psychiatric hospital: Firstly,On the basis of consulting a large number of relevant literature,8 medical staff,4 patients and 4 family members of a psychiatric hospital in Chengde were interviewed to conduct semi-structured interviews to form the initial questionnaire.Then through the convening of the expert meeting and the pre survey to amend the questionnaire,and test its reliability and validity.Eventually form the doctor-patient relationship status and influencing factors questionnaire in psychiatric hospital.2.Investigation into the current situation of the doctor-patient relationship in psychiatric hospitals in Hebei Province and analysis of its influencing factors: 431 medical staff and 438 patients in psychiatric hospitals of Hebei Province were investigated by using stratified sampling method.Applicate statistical description and t test to understand the current situation and the influencing factors of the doctor-patient relationship in the psychiatric hospitals of Hebei Province.Results:1.The relationship between doctors and patients in psychiatric hospital was investigated: The questionnaire consists of three parts: The first part is the general information questionnaire,including the general information of medical staff and patients.The second part is the questionnaire of doctor-patient relationship,including 5 items;The third part is the questionnaire of influencing factors of doctor-patient relationship,including 3dimensions,including the environmental factors(9 items),staff factors(12items)and patient factors(9 items),a total of 30 items.The Cronbach’s alpha coefficient of the doctor-patient relationship questionnaire is 0.832,the Cronbach’s alpha coefficient of each dimension was 0.801~0.843,the test-retest reliability was 0.889;The S-CVI values of questionnaire were 0.994,the dimension of the average CVI value was 0.983~1.000;The exploratory factor analysis showed that 3 factors were extracted from the questionnaire,the cumulative explanatory variable was 55.351%,and the factor load of each item was more than 0.4.2.Investigation into the current situation of the doctor-patient relationship in psychiatric hospitals in Hebei Province and analysis of its influencing factors: 920 respondents including 460 medical staff and 460 patients was surveyed in this study within which valid questionnaires of medical staff and patients were 431 and 438,qualified rate of their questionnaires were 93.7% and 95.2%.Survey results show the cognitive scores(item score)of the status of the doctor-patient relationship from the medical staff and patients were respectively(3.11±0.47)and(3.51±0.55).The attitude of medical staff to the relationship between doctors and patients is not as good as that of patients(P < 0.01).It’s found by analyzing the influencing factors that the medical staff’s professional and humanistic quality should be improved.Psychiatric hospitals in Hebei province lack of funds,equipments and staff.Patients’ family members have heavy economic burden.Social news or media bias.Our society needs to give publicity to the knowledge of mental health and respect,understand,and support patients with psychosis.Conclusion:1.The questionnaire has good reliability and validity,which can be used to investigate the current situation and influencing factors of doctor-patient relationship in psychiatric hospital.2.The overall situation of the relationship between doctors and patients in psychiatric hospitals of Hebei Province is acceptable,but it still need to be improved.It is suggested that the psychiatric hospital of Hebei province should strengthen the re education of medical staff,improve the professional and humanistic quality of medical staff,and enhance the respect,understanding and support for patients with mental illness.The government should increase the investment in psychiatric hospitals,improve medical equipment.Improve the allocation system of medical human resources,alleviate the shortage of medical staff.The government should increase the proportion of reimbursement for medical treatment,to alleviate the problem of expensive patients.The news media should report the medical environment objectively and spread positive energy.The society should strengthen the propaganda of mental health knowledge,and enhance the public’s understanding of the Department of psychiatry.
