PartⅠ Study On the Correlation Between Behcet’s Disease and Spondyloarthropathies Objective:To determine the frequency of sacroiliitis or spondylitis in our region,and the correlation between Behcet’s Disease(BD)and Spondyloarthropathy(Sp A),we followed up the disease history and routinely performed the axial joints CT scan in the 192 patients with BD.Method:We first retrospectively analyzed the data of clinical symptoms,laboratory tests,and sacroiliac joint’s CT scan in 192 cases of BD patients who were treated in our hospital and determined the frequency of sacroiliitis or spondylitis.For those lacked of the records of axial arthritis,we re-inquired the disease history of axial arthritis and routinely checked sacroiliac joints by CT scan.Last we compared the changes of axial arthritis frequency before and after follow-up.ASAS and Amor criteria were used for the diagnosis of Sp A in BD patients.Results:A total of 192 patients with BD were enrolled at the hospital from January 2010 to June 2015.Among them,28 cases showed sacroiliitis before follow-up(14.58%)and increased to 112 cases after(58.33%,P<0.001).Besides sacroiliitis,192 cases had recurrent oral aphthous ulcers(100%),111 cases skin lesions(57.8%),117 casesgenital ulcerations(60.9%),70 cases ocular symptoms(36.5%),78 cases peripheralarithritis(40.6%),76 cases axial skeleton involvement(39.6%),and 35 cases heel pain(18.2%).All 192 cases showed RF negative(100%),188 cases ANA negative(97.9%),and3 cases HLA-B27 positive(1.6%).The 112 cases were found different degree of pathological changes of sacroiliac joint by CT scan.Consistently,the frequency of BD patients with Sp A increased from 3.6%(ASAS)or 2.6%(Amor)before to 42.19%(ASAS)or 29.69 %(Amor)after follow-up(P<0.001).Conclusion:The frequency of Sacroiliitis and Sp A was increased by following up disease history and routinely examining sacroiliac joint by CT scan.Moreover,the frequency of sacroiliitis and Sp A in BD patients in our region was significantly higher than that in other regions reported before.All above suggest that missed diagnosis is also a possible reason for the lower frequency of axial arthritis and the BD patients in our region may have more severe manifestations.PartⅡ The Detection of HLA-B51 in Behcet’s Disease Objective:Detecting the frequency of HLA-B51 in Behcet’s Disease,and analysing the correlation of HLA-B51 and clinical manifestations of Behcet’s Disease.Method:Genomic DNA form samples was extracted from ethylenediaminetetraacetic acid(EDTA)-anticoagulated peripheral blood using a commercial DNA purification kit(SE Blood DNA Kit,D3471-01,Omega,USA).Rapid detection of HLA-B51 by real-time polymerase chain reaction and high-resolution melting analysis.Analysing the correlation of HLA-B51 and clinical manifestations of Behcet’s Disease.Results:The frequency of HLA-B51 of BD in Shanxi province was 31.43%,the control group was 11.76%,and the difference was statistically significant.Pure BD patients was 33.33%,BD merging Sp A patients was 28.57%,there was no statistically significant.Analysis results indicated that opthalmitis,skin injury and acupuncture reaction had greater dependence in HLA-B51.Addition,male was a risk factor for BD patients.Conclusion:The frequency of HLA-B51 of BD in Shanxi province was higher than the control.HLA-B51 was associated with vasculitis,not arthritis.
