Study On Equity Of Health Resource Allocation In Ningxia

Background: Since the new medical reform in 2009,the government has paid more attention to the health care,established a relatively sound basic medical system,and invested heavily in the development of health undertakings.However,even to this day,the difficulty of expensive medical treatment problem hasn't been significantly improved,and the contradiction between the people's growing needs of health care and limited quantity of health resources is still outstanding.Thus,the fairness and reasonability of health resource allocation is the key issue and subject that should be urgently researched and solved in China's health care development and reform.Objective: Collecting the quantity of health resources allocation of Ningxia from Health statistics yearbook and Health Development Communique of Ningxia and China,and combining the development of social economy and health status,this paper use economic approach to analyze the fairness of health human,material and financial resources allocation to understand its main problem and make reasonable suggestions about the new medical reform for the governmental decision-making departments.Methods: Using despritive methods to analyze the status of health resources in Ningxia.Using Lorenz curves,Gini coefficient and Theil index to deeply analyze the equity of population and geography allocation of health human,material and financial resources in Ningxia from 2009 to 2015.Using Analytic Hierarchy Process determine the heavy weight of all health resource index,then comprehensive assess health resources allocation of five cities combining Technique for order preference by similarity to ideal solution..Results:1.Health human resource: during six years,the total amount of health worker rose steadily,increased by 15813 and mainly focused on health technician.The Gini coefficient of health human resource by population was less than 0.40 and showed a downward trend,the equity of population allocation was better.However,Gini coefficient by geography was more than 0.40 and showed rising trend,the equity of geography allocation was worse,especially nurse and manager.The contribution of differences within regions to the total Theil index was more than 51.00% and higher than the inter-regional differences.2.Health material resource: the total amount of various health institutions was up trend.The total amount of health institutions and beds were respectively 4289 and33804 in Ningxia by 2015.And Gini coefficient by population of both them was less than 0.20 and the geography was less than 0.40.The equity of population allocation was better than geography allocation.The contribution of differences within regions to the total Theil index was more than 62.00% and obviously higher than the inter-regional differences.3.Health financial resource: 2009-2015,the share of total expenditure on health in GDP increased from 6.32% to 7.89% and it rose steadily yearly.The growth rate of government budgetary expenditure on health was higher than GDP and national level.Gini coefficient by population of health financial subsidy was less than 0.20 and geography was less than 0.40.The equity of population allocation was better than geography allocation.4.Comprehensive assessment: 5 cities in Ningxia had the Ci value of 0.1582-0.6904.The difference of health resource allocation in Ningxia was great.And the worst allocation was Zhongwei city(0.1582)and the best was Yinchuan city(0.6904).Except Yinchuan and Shizuishan city,the Ci values of Wuzhong,Guyuan and Zhongwei city increased in six years.Conclusions:2009-2015,the total amount of health resources increased yearly and the average possession of health resources in Ningxia was higher than the national average level on the whole.However,there existed regional differences and the total amount of nurse and manager was relatively small.The equity of health human,material and financial resource allocation was good and improved gradually on the whole and their equity of population allocation was better than geography allocation.by gene coefficient analysis.Meanwhile,the equity of health human,material and financial resource allocation was good on the whole and their equity was mainly influenced by the regional health resources allocation by theil index analysis.Moreover,the health resources in descending order of weight are health human resource,financial resource and material resource by analytic hierarchy process.Finally,the comprehensive rank of Ningxia's health resource allocation in descending order are yinchuan,shizuishan,guyuan,wuzhong and zhongwei by topsis.Suggestions:1.Scientifically develop regional health planning to promote the integration of health resources optimization.2.Heavily increase financial input to improve the basic medical health service system.3.Actively take various measures to establish effective development mechanism of health human resource.4.Center on the practical needs to improve the equity of health resource allocation.5.Based on the patients actual demand to rationally allocate health resources.6.Foucs on difficult and remote rural areas to promote the coordinated development of health work in the region.
