Objective: To investigate the efficacy of the combination of Epirubicin(EPI)and Mannatide with different mechanisms in the irrigation treatment of bladder cancer after operation. Methods: A total of 92 patients were randomly divided into three groups according to the order of treatment: Group A(n = 29)was treated with 40 mg of EPI dissolved in 30 ml of 0.9% sodium chloride injection;Group B(n = 32)was treated with 40 mg Mannatide dissolved in 30 ml of 0.9% sodium chloride injection;Group C(n=31)was treated with 40 mg EPI and 40 mg Mannatide dissolved in 30 ml of 0.9% sodium chloride injection.Transurethral resection of bladder tumor(TURBT)was performed in all patients. One week after the operation, perfusion treatment was began and implemented regularly by one person and the procedure was performed strictly according to the principle of aseptic. The recurrence and adverse reaction rates were compared among the three groups through follow-up.Results: Every patient was followed up for one year. The recurrence rates of bladder cancer in group A,B and C were 31.03%(9/29),34.38%(11/32)and 3.22%(1/31)respectively.The statistical analysis showed that there was no significant difference between group A and B(P>0.0125),while group C and group A or B,the difference was statistically significant(P<0.0125).The incidence of adverse reaction of bladder irritation in the three groups was 48.28%,3.13% and 16.13% respectively. There was significant difference between group A and group B or C(P <0.0125),and the difference between group B and C was small without any statistical significance(P>0.0125).The incidence of gross hematuria was 17.24%,0%,6.45% respectively. The differences between the three groups were statistically significant. Conclusions: The combination of EPI and Mannatide is superior to single EPI or Mannatide in the prevention of postoperative recurrence of urinary bladder cancer and is safe and effective. It is worthy of clinical application. There was no significant difference in the antitumor effect between EPI and Mannatide, but the adverse effects of EPI were significantly higher than those of Mannatide.Long-term efficacy remains to be further explored because of finite time.
