Objective: The objectives of this study were to investigate the value of the serum pepsinogen(PG)test for the prediction of early-stage gastric cancer and to make the optimal cutoff value for the early-stage gastric cancer detection in Qinghai Province by detecting the serum levels of PG in patients with non-atrophic gastritis(NAG),chronic atrophic gastritis(CAG),early-stage gastric cancer(EGC)and advanced-stage gastric cancer(AGC).Methods: A total of 199 participants were enrolled in Qinghai provincial people’s hospital from 2014.11 to 2016.05.Including 47 patients with NAG,62 cases of CAG,30 cases of EGC and 60 cases with AGC by endoscopy and histological diagnosis.The serum levels of PG I and PG II in each group were measured using enzyme-linked immunosorbent assay(ELISA),and the expressive differences of PG in each group were analyzed.The PG optimal cutoff value for EGC detection can be demonstrated by Receiver operating characteristic curve(ROC).Results:1.The levels of PG I were significantly different among the four groups(P <0.05);Compared with NAG group,the levels of PG I in CAG group,EGC group and AGC group were significant lower(P <0.05);The concentration of PG I in AGC group were lower than those in NAG group and EGC group(P <0.05).2.The levels of PG II were significantly different among the four groups(P <0.05),the levels of PG II in EGC group and AGC group were higher than those in NAG group and CAG group(P <0.05).3.The levels of PGR were significantly different among the four groups(P <0.05),the levels of PGR in CAG group,EGC group and AGC group were lower than those in NAG group(P <0.05);Compared with CAG group,EGC group and AGC group were significantly lower(P <0.05);Compared the level of PGR with EGC group,AGC group decreased significantly(P <0.05).4.The PG optimal cutoff value for EGC detection which demonstrated by ROC is PG I 82.25μg/L,PGR 3.09.Conclusion:Serum PG I and PGR are of great value in the detection of EGC.In the identification of a high risk group for gastric cancer in Qinghai Province,the optimal cut-off value of PG I and PGR are suggested 82.25μg/L,3.09 respectively.
