Objective:The purpose of the study was to research the applicability of measuring serum pepsinogen(PG) and gastrin-17(G-17) as screening tests for gastric cancer and atrophic gastritis.Methods:A total of 133 subjects with gastric disease were enrolled in the second hospital of shanxi medical university from February 2014 to December 2014.Of those cases,44 patients were diagnosed with non atrophic gastritis, 47 patients were chronic atrophic gastritis,42 were gastric cancer.All subjects underwent a gastrocopy and biopsy.Approximately 5 ml fasting blood was collected from each participant, then serum PGI,PGII and G-17 levels were detected by Biohit ELISA kit(Finland), and PG I/II ratio was calculated. Anti-HP antibody was detected by Helicobacter pylori urease antibody diagnostic kit(colloidal gold) and RUT. Date was analyze by SPSS for windows version16.0 using analysis of variance(ANOVA). Diagnostic accuracy of biochemical tests was analyzed using receiver operating characteristic(ROC) curves. The significance level was set at 0.05.Results:The levels of PG I and PGR in gastric cancer and chronic atrophic gastritis groups were significantly decreased than that of non atrophic gastritis groups. The level of G-17 in gastric cancer were significantly increased,while that of chronic atrophic gastritissignificantly decreased.For the best discrimination of gastric cancer, the cut-of values of PG I, PGR calculated by receiver operating characteristic(ROC)curve were PGI<74ng/ml(sensitivity: 85.7%, specificity: 75%) and PGR<4(sensitivity: 89%,specificity:62.5%). As for chronic atrophic,the cut-of values of PG I, PGR and G-17 calculated by receiver operating characteristic(ROC)curve were at PGI<91ng/ml(sensitivity: 77.3%, specificity:50.0%), PGR<9(sensitivity: 72.7%, specificity:53.2%),G-17<6.0pmol/L(sensitivity: 65.9%,specificity: 63.8%). The level of PGI and PGII were significantly increased in HP positive group than those of HP negative group, while the level of PGR decreased.Conclusions:Serological methods can be used to screen gastric cancer and chronic atrophic gastritis.
