Objective:To retrospectively analysis the clinical characteristics,the results of laboratory examination and the situation of post-discharge of bullous pemphigoid(BP).To explore the better methods for dignosis and treatment of bullous pemphigoid.Methods: Clinical datas of 122 patients with BP from January 2009 to December 2016 in the department of dermatology,the Fourth Hospital affiliated to He Bei Medical University were retrospectively analysed.SPSS21 was used to analysed the groups of data.Results:68 male patients and 54 female patients in this study.Male to female rato is 1.26:1.The onset age was 6-94 years old,the average age was68.82±15.58 years old,patients over the age of 60 accounted for 79.5% of the total number of patients.The average age of male patients was 71.46±13.85 years old,the average age of female patients was 65.60±17.07 years old.Most patients with no obvious cause before onset.The eruption of BP was pleomorphism.In 122 patients misdiagnosed rate:50%.Nikolsky in 112 patients were negative(91.8%).Mucosal lesions occurred in 32 patients(26.23%).In this study,a total of 98 patients(80.33%)accompanied with one or more other system diseases,including hypertension 52 cases,nerrous system diseases 43 cases,malignant tumor 7cases and diabetes mellitus15 cases.Detection of patients with BP180 antibody were 117 cases,99 cases were BP180 positive(84.62%).Albumin deficiency was the most common phenomena.The patients were mainly treated with glucocorticoid or the combination of glucocorticoid with immunosuppressive agents.Patients with poor or unacceptable response to general therapy treated with plasma exchange.Among all patients,59 cases ended with cure,51 cases were under control,the total rate of control was 90.1%,12 cases were unhealed.83 patients were followed up by telephone,32 cases were regular outpatient follow-up(4cases were recidivation),18 cases were unregulated(5 cases were recidivation),35 cases were death(included 2cases were unregulated and recidivation).Conclusions:1 BP primarily affects old people,especially 60 years or older.Paients with BP often accompanied by one or more complications,diseases of the cardiovascular system and nervous system diseases are most common.2 Serum titer of anti-BP180 autoantibody is well correlated with disease severity in BP patients.3 Plasma exchange is a useful adjuvant treatment in BP patients.4 The starting dose of glucocorticoid in mucosal involvement patients is higher,and the hospital day is longer.5 The recurrence rate of regular outpatient follow-ups is lower than the unregulated ones.
