| Objective:To detect the level of anti-BP180 Ig E antibody,serum total Ig E,and eosinophilic cationic protein(ECP)in the serum of patients with bullous pemphigoid(BP),also observe the levels of peripheral blood eosinophils,anti-BP180 Ig G antibody and BPDAI scores,to explore the expression of serum anti-BP180 Ig E antibody,peripheral blood EOS,and their correlation with the clinical features of BP.Methods:From January 2018 to February 2021,31 BP patients with newly diagnosed BP in the Department of Dermatology,the Fourth hospital of Hebei Medical University were selected as BP group.Meanwhile,31 healthy people with the gender and age matched with BP group were collected and divided into NC group.The general data,peripheral blood eosinophils and serum samples of patients in BP group and NC group were collected,and the serum levels of anti-BP180 Ig E antibody,ECP and total Ig E were detected.In BP group,anti-BP180 Ig G antibody levels and BPDAI scores were collected.SPSS21.0 software was used to analyze the data.Mann Whitney U test was used to compare the data of the two groups,and Spearman correlation analysis was used to analyze the correlation between the data.Results:1 Clinical data analysis1)General:Among the 31 BP patients,18 were male(58.06%),13 were female(41.94%).The ratio of male to female was 1.38:1.The age range of BP group was 9-87 years old,and the mean was(64.84±18.747)years old.There was no statistical difference in age and gender between BP group and NC group(P>0.05).2)The peripheral blood eosinophils:Among the 31 patients in BP group,12 patients(38.71%)had elevated peripheral EOS counts(>0.52×10~9/L),13patients(41.94%)had normal peripheral EOS counts,and 6 patients(19.35%)had decreased peripheral EOS counts(<0.02×10~9/L).3)BPDAI scores:Among 31 BP patients,the BPDAI score range was11-137,with an average of(54.06±29.57).4)The peripheral blood eosinophils were positively correlated with the total BPDAI scores(r_s=0.407,P=0.023)and urticaria/erythema scores(r_s=0.490,P=0.005),but not with blister/erosion(r_s=0.249,P=0.177)and pruritus scores(r_s=0.151,P=0.417).2 Experimental results2.1 The results of serum anti-BP180Ig E antibody in BP group and NC group.1)In the BP group,there were 17 patients(54.83%)with serum anti-BP180Ig E antibody positive.and the NC group had no patients were positive for anti-BP180 Ig E antibody.2)In BP group,serum anti-BP180 Ig E antibody levels were positively correlated with total BPDAI scores(r_s=0.540,P=0.025)and urticaria/erythema scores(r_s=0.674,P=0.003),but not with blister/erosion scores(r_s=0.358,P=0.158)and pruritus scores(r_s=-0.098,P=0.71).3)In BP group,the serum anti-BP180 Ig G antibody levels were positively correlated with total BPDAI scores(r_s=0.812,P<0.01)and erosion/blister score(r_s=0.695,P<0.01),but not with urticaria/erythemascores(r_s=0.481,P=0.31)and pruritus scores(r_s=0.294,P=0.108).2.2 The results of serum ECP and total Ig E in BP group and NC group.The serum ECP levels in the BP group(17.30±21.89 ng/ml)were significantly higher than that in the NC group(4.76±2.35 ng/ml),and the difference was statistically significant(P<0.01).The serum total Ig E levels in BP group(419.91±316.76 ng/m L)were significantly higher than that in NC group(129.68±62.63 ng/m L),and the difference was statistically significant(P<0.01).Conclusions:1.There were anti-BP180 Ig E antibody in peripheral blood of some BP patients.2.The levels of anti-BP180 Ig E antibody and anti-BP180 Ig G antibody in the serum of BP patients can reflect the severity of the disease.Anti-BP180Ig E antibody was associated with erythema/urticaria manifestations,and anti-BP180 Ig G antibody was associated with blisters/erosion manifestations.3.About one third of BP patients had increased EOS count,which can reflect the severity of the disease and was related to urticaria/erythema manifestations.4.The levels of serum anti-BP180 Ig E antibody and the EOS levels in peripheral blood of BP patients were positively correlated with urticaria/erythema manifestations,suggesting that both of them may be involved in the formation of urticaria/erythema like lesions.5.There was no correlation between pruritus and anti-BP180 Ig E antibody,anti-BP180 Ig G antibody and peripheral blood EOS counts. |
