| Objective: Coal workers’ pneumoconiosis(CWP)had showed a progressive disease due to the lack of early clinical diagnosis,effective treatment measures and other issues.This study was based on the analysis of CWP patients’ age,dust exposure time,pulmonary function and other indicators,and the expression levels of CD3~+,CD3~+CD4~+,CD3~+CD8~+,CD4~+CD25high CD127 low cells in peripheral blood were detected.The correlations between these factors were explored in order to provide new ideas and theoretical reference for the pathogenesis and prevention of CWP disease.Methods: According to the GBZ 70-2009 “diagnostic criteria of pneumoconiosis”,111 cases of CWP patients were enrolled in this study.According to the severity of disease,patients were divided into CWP I group(30 cases),CWP II group(60 cases)and CWP III group(21 cases).The CWP stage was independently determined by the Pneumoconiosis Panel at the same time,30 healthy cases were taken as control group.Patients with coronary heart disease,cerebrovascular disease,and diseases of hypertension,diabetes,recent pulmonary infection,severe liver and kidney dysfunction,recently used immunosuppressive agents were excluded.All participants enrolled in the study signed the consent form.The pulmonary function,CD3~+、CD3~+CD4~+,CD3~+CD8~+ and CD4~+CD25highCD127low cells in peripheral blood were detected in all the participants,and the correlations between these factors were analyzed.All the data involved in this study were analyzed by SPSS 19 statistical software.The t-test was used to compare the two means,and the correlation was calculated by Pearson test and the correlation coefficients were calculated.All the hypotheses were tested by bilateral test,P<0.05 was considered as statistically significant.Results:1 VC,FEV1/Pre,FEV1/FVC and TLCO/VA of CWP patients were significantly lower than the healthy control group(P<0.05).With the increase of CWP stage,VC 、 FEV1/Pre,FEV1/FVC and TLCO/VA were gradually decreased(P<0.05);2 Compared with the control group,the levels of CD3~+ and CD3~+CD8~+cell level in CWP group were gradually decreased(P<0.05);CD3~+CD4~+ cells,CD3~+CD4~+/CD3~+CD8~+ ratio and CD4~+CD25highCD127low cell level were gradually increased(P<0.05);3 The correlation analysis showed that CD3~+CD4~+ cell level,CD3~+CD4~+/CD3~+CD8~+ ratio and CD4~+CD25highCD127low cell level were negatively correlated with VC,FEV1/Pre,FEV1/FVC(P<0.05);CD3~+CD8~+cell level were positively correlated with VC,FEV1/Pre,FEV1/FVC,TLCO/VA(P<0.05);CD3~+CD4~+/CD3~+CD8~+ ratio was negatively correlated with TLCO/VA(P<0.05),CD3~+ cell level was positively correlated with TLCO/VA(P<0.05);CD3~+CD4~+ cell and CD4~+CD25highCD127low cell levels were not correlated with TLCO/VA(P>0.05).CD3~+ cell level was not correlated with VC、FEV1/Pre and FEV1/FVC(P>0.05).Conclusions:1 With the increase of age and dust exposure time,the CWP stage and the severity of the disease were increased;2 With the progression of CWP,pulmonary function of patients showed restrictive and mixed ventilation dysfunction,pulmonary diffuse function were also significantly reduced,The early changes of the pulmonary function could sensitively reflect the progress of CWP,and the monitoring of pulmonary function changes could provide early warning for the diagnosis of CWP;3 With the increase of immune regulation and immunosuppressive capacity,immune system environment disorder and the severity of the disease were also increased,suggesting that immune T cells disorder in CWP patients might be an important pathogenesis of CWP;4 The level of T cells in peripheral blood was significantly correlatedwith pulmonary function.The expression level of T cells in peripheral blood could reflect the degree of injury of pulmonary function and assess the development of the patient’s condition. |
