| Objective By comparing the elderly with or without periampullary diverticula and different types of relationship with common bile duct stones,analysis and study of periampullary diverticula of the elderly and typing of common bile duct stones formation,size and impact of relapse.Methods Retrospectively analysis all patients data undergoing ERCP in diagnosis and treatment from January 2014 to December 2016.According to the inclusion and exclusion criteria,1007 cases(accounted for 66%)patients were selected.310 cases were checked out periampullary diverticula,were divided into diverticulum group,which male 164 cases,female 146 cases,average age(74.51 ± 7.19);And according to the different locations of PAD were divided into three type: type I,papilla located inside the diverticulum;type II,papilla located at the edge of the diverticulum;type III,papilla located closer than 3 cm to the diverticulum.Remaining 697 cases is divided into non-diverticulum group,which male 350 cases,female 347 cases,average age(72.89 ± 7.35).The effect of duodenal diverticula and typing on common bile duct stones,mean CBD stone size,recurrence of CBD stones and gallstone associated with CBD stones and CBD stones after cholecystectomy were analyzed in elderly patients.Results 1、The detection rate is 30.8%(310/1007)of PAD in elderly patients,and with increasing age,the detection rate has increased in this research;2、PAD and different type in elderly patients are not associated with gender;3、With or without PAD in elderly patients is no differences in overall symptoms;4、The prevalence were higher in elderly patients with PAD than without PAD on CBD stones,recurrence of CBD stones and gallstone associated with CBD stones and CBD stones after cholecystectomy(P value respectively for < 0.001,and <0.001,and 0.019,and 0.026);5、The prevalence was higher in elderly patients with PAD type II than in those with PAD type I and type III on CBD stones(P=0.023);6、The recurrence of CBD stones,no significant differences between the three components(P=0.107);7 、 Gallstone and PAD were independent risk factors for formation of Choledocholithiasis and recurrent.Conclusions Periampullary diverticula in this research is one of high-risk factors for formation of Choledocholithiasis and recurrent,and II type is more effect than other type on formed of CBD stones. |
