Genetic Association Of HLA-DRB1 Multiple Polymorphisms With Dermatomyositis In Chinese Population

Objective: idiopathic inflammatory disease(IIMs)is a kind of autoimmune disease,which is not very clear.in pathogenesis.Genetic variability in human leukocyte antigen(HLA)plays an important role in the pathogenesis of dermatomyositis(DM).Human leukocyte antigen(HLA,human antigen)class II gene region includes nearly 30 loci,of which the classical class II genes generally refer to DR,DP and leukocyte,and DQ.In recent years,have found some class II genotype associated with DM in different populations and specific antibody in sera of patients with DM expression,such as anti-melanoma differentiation associated 5(melanoma differentiation associated gene 5)antibodies and HLA class II genotype correlation.The aim of this study was to investigate the association of HLA class II with Chinese DM patients.Method: 224 DM patients and 300 healthy controls were enrolled.High resolution typing of HLA-DRB1 alleles was performed by sequencing based typing(SBT).The HLA-DQA1 and HLA-DQB1 alleles were determined by PCR sequence-specific primed(SSP).Result: The frequencies of HLA-DRB1*0901(28.6%vs.11.3%,P< 0.0001,OR = 0.31,95% CI = 0.25-0.40)and HLA-DRB1*1201(29.0% vs.11.0%,P< 0.0001,OR = 0.30,95% CI = 0.24-0.39)in DM patients were significantly higher than that in healthy controls.No significant difference was found in HLA-DQA1 or DQB1 alleles between DM patients and healthy controls.Furthermore,DM patients with anti-melanoma differentiation-associated gene 5 antibody(anti-MDA5)had a significantly higher frequency of HLA-DRB1*1201 compared to that for patients without anti-MDA5(P < 0.0001,OR = 4.77,95% CI: 2.29-9.93).HLA-DRB1*0901 was a poor prognostic factor(P = 0.01,OR = 9.21,95% CI: 1.47-57.50)for DM patients with anti-MDA5 autoantibody.Conclusion: In summary,our findings indicate that HLA-DRB1*0901 and HLA-DRB1*1201 genotypes may contribute to susceptibility of adult DM in Han Chinese population.In addition,the DRB1*1201 genotype is significantly associated with Anti-MDA5 antibody in DM patients.
