ObjectiveThrough the epidemiological survey ofalcoholic liver diseaseamongstaffs in Yanchang oil field,to learn about the prevalence ofalcoholic liver disease and its correlate risk factors.MethodsOpt Yanchang oil field Co.Ltd.as for the survey area.Using a cluster sampling method to collect oil field workers' health examination data in June 2016,all accept somatology anthropometric measurement(height,weight,blood pressure),laboratory tests(blood routine,liver and renal function;blood glucose,lipids;viral markers)and ultrasonic inspection,and fill out the complete questionnaire survey,the complete date was 6723 copies.The data were input into database with SPSS19.0 software,and carried out descriptive analysis,measurement data statistical description using(x ąs),count data using rate,data analysis was performed using the chi-square test,and logistic regression model.Results1.There were 3667 people being used to drinking,the drinking rate is 54.54%.There were 397 cases of ALD in the crowd,the prevalence was 5.91%.2.The result of the prevalence of ALD in different age paragraph displayed 50-59 years old was peak in the crowd,the prevalence of ALD was peak at the age of 60 years in drinking people.The prevalence increased with age,and the difference was statistically significant(P<0.001).No matter men and women,the prevalence of ALD was peak at 40-49 years old.Men's prevalence of ALD was higher than women(P<0.001).3.There were 3441 drinkers in 5288 male group,the drinking rate was 65.07%;There were 226 drinkers in 1435 female group,the drinking rate was 15.75%;Men's drinking rate and prevalence of ALD were higher than the female(P<0.001).Men was the major group of alcohol consumption.4.The education of drinkers showed that the number of junior high school accounted for most,proportion was 42.40%.And the others were high school,in order,primary school and below,college,university or above,proportion was 26.70%?23.40%?4.91%?2.59%.5.The types of drinking showed that beer was the first choice,accounting for 35.70% of the total number of drinkers,followed by white wine,accounted for 32.45%.The beer and liquor was the mainstream of alcohol consumption products.6.There were 13.14% had the habit of drinking on empty stomach in the group of drinkers,The drinkers who choosed drinking with meals accounted for 86.86%.The prevalence of ALD of drinkers on an empty stomach was 18.05%,and the other was 9.73%,the prevalence of drinkers on an empty stomach had the higher prevalence,the difference was statistically significant(P < 0.001).The prevalence of drinkers that drinking mixed and pure white wine was higher.7.The result showed that,with the increase of the alcohol level and drinking years,the prevalence of ALD was greatly increasing,the differences of prevalence of ALD were statistically significant(P < 0.001).Drinking more than 20 g per day,drinking more than 5 years,the prevalence of ALD was increasing.And drinking more than 40 g per day,drinking more than 5 years,the prevalence of ALD was greatly increasing.8.It showed that the abnormal rate of liver function index(?-GGT?ALT?AST)increase with the level of alcohol,and the differences was statistically significant(P < 0.001);Compared with the group of drinking less than 20 g,the abnormal rate of liver function index(?-GGT?ALT?AST)of the rest three groups of was statistically significant(P < 0.001).As the growth of and drinking years,a the abnormal rate of liver function index(gamma?-GGT?ALT?AST)gradually increased,difference of the abnormal rate of ALT and AST was no statistically significant(P > 0.05),the abnormal rate of ?-GGT was statistically significant(P < 0.001);Compared with the group of drinking less than 5 years,the difference of abnormal rate of thedrinking more than 20 years group was statistically significant(P < 0.001),the rest had no statistical significance(P > 0.05)9.Physical examination population can be divided into there groups,respectively were ALD group,drinking group and the control group(not drinking),the result showed that the abnormal rate of liver function,blood pressure,blood fat,BMI,MCV in three groups were statistically significant(P < 0.001).Compared with the control group,the abnormal rate of blood fat had no statistical significance in ALD group(P > 0.05);The differences of the rest of the test index were statistically significant(P < 0.001).To compare with ALD group and drinking group,the abnormal rate of blood fat and blood pressure had no statistical significance in ALD group(P > 0.05),and the rest test indicators were statistically significant(P < 0.001).10.The multiple factors Logistic regression model showed that age,male,obesity,drinking,drinking pattern,cultural degree,?-GGT,AST,MCV is closely related to the occurrence of ALD(P<0.05).Conclusion1.The prevalence of ALD was 5.91% among staffs in Yanchang oil field,the rate of drinking was 54.54%,the rate of alcohol was 14.55%.2.The prevalence of ALD was greatly increasing with age,the prevalence of ALD was peak at 50-59 years old;The age of first drinking was lower,and the education of the adolescents is especially important.3.With the increase of the alcohol level and drinking years,the prevalence of ALD was greatly increasing.4.Men's drinking rate and prevalence of ALD were higher than the female.Men was the major group of alcohol consumption.There is a high prevalence of ALDamong staffs in Yanchang oil field.Gender,age,education,alcohol intake,drinking on an empty stomach and obesity are the major risk factors of this disease.5.The beer was the first choice type of drinking,and the beer and liquor was the mainstream of alcohol consumption products in the survey area.Drinking with meals was the major drinking pattern.The prevalence of drinkers that drinking mixed and pure white wine was higher,especially the drinkers on an empty stomach had the highest prevalence.6.The education is closely related to the occurrence of ALD.The number of junior high school accounted for most,and the prevalence of ALD was the highest.The education is particularly important.The propaganda and education of health drink in the survey area is particulary important.7.The prevalence of ALD of obesity was higher.8.The abnormal rate of liver function,blood pressure,blood fat,BMI,MCV in three groups had significant difference.9.The Logistic analysis showed that age,male,obesity,drinking,drinking pattern,cultural degree,?-GGT,AST,MCV is closely related to the occurrence of ALD(P<0.05).
