Objective We initially establish a database of stroke in Gansu Province,to save and manage the clinical data of stroke.Then we discuss the characteristics of the risk factors and etiological classification of young ischemic stroke,and explore the application values of the database.Methods1 By understanding the research status and progress of stroke in domestic and international,combining with the regional characteristics of stroke and its high risk population in Gansu Province,we set the primary field,and then establish the stroke database in Gansu Province with the help of my structured query language(MySQL)database management software.2 Part of the ischemic stroke patient’s clinical data of the department of neurology in Lanzhou University Second Hospital were pre-registered by the stroke database in Gansu Province.A total of 52 young patients and 53 middle-aged and old patients with ischemic stroke were included,and the clinical data of all the subjects were derived and analyzed statistically.Based on the Chinese Ischemic Stroke Subclassification(CISS),we clarify the etiology classification of all subjects and compare the difference between the two groups in the characteristics of the etiology classification and the related risk factors.Results1 We initially established a database of stroke in Gansu Province.The database contains five data tables: general information table,stroke area table,stroke type table,risk factors table,means of intervention table and case data table.This is a database with the function of entry,query,import,export,data backup and porting.2 There were 52 cases in young ischemic stroke group,and the average age is(36.94±8.55)years,43(82.69%)male datas and 9(17.31%)female ones.Young ischemic stroke risk factors of the top 4 are male 82.7%,smoking history 53.8%,hypertension 44.2%,hyperlipidemia 42.3%.In contrast,the middle-aged and old group is male 73.6%,hypertension 64.2%,hyperhomocysteinemia 41.5%,diabetes 28.3%.Comparison of risk factors between the two groups,there was significant difference between two groups with hyperlipidemia,hypertension,smoking,drinking,family history of stroke,intracranial and extracranial arterial stenosis(P<0.05).Hyperlipidemia,smoking,drinking,family history of stroke,intracranial and extracranial arterial stenosis in the young groups was remarkable higher than that in the control group,with significant difference(P<0.05).The hypertension in middle-aged and elderly group was significantly higher than that in young group,the difference was significant(P<0.05).Meanwhile,the CISS classification in young group was: large artery atherosclerosis(LAA)28.8%,other etiology(OE)21.2%,penetrating artery disease(PAD)19.2%,undetermined etiology(UE)17.3%,cardioembolism stroke(CS)13.5%.There was no significant difference between the two groups except the OE type.The classification of LAA,CS and PAD in the middle-aged and old ischemic stroke group ware higher than that in the young ischemic stroke group.The OE and UE type in the young ischemic stroke group ware higher than that in the middle-aged and elderly ischemic stroke group.Conclusions1 The stroke database in Gansu Province has the following characteristics: complete content;reasonable structure;clear hierarchy;simple operation.The database plays an important role in the study of disease etiology,risk factors,epidemiology,prevention and treatment,and so on.2 Young ischemic stroke patients used to appear mostly in male.Smoking history,hypertension,hyperlipidemia are the most common risk factors in young ischemic stroke patients.Health care programs targeting the prevention and treatment of these factors will reduce the associated morbidity and mortality of stroke among this socioeconomically active age group.The Chinese Ischemic Stroke Subclassification is critical in conducting basic research and clinical practice,it is important to identify the etiology and implement preventive measures of young ischemic stroke. |
