Objectives It’s still unclear about the relationship between periodontitis and osteoporosis according to the domestic and foreign literature. The purpose of this study is to find the best evidence system to evaluate the relationship between periodontitis and osteoporosis. Methods We searched literatures on periodontitis and osteoporosis published in Pub Med,The Cochrane Library,CBM,CNKI,and Wanfang database from 1989 to 2016.Language is English and Chinese.Accordingto the inclusion and exclusion criteria, two researchers respectively did the literature screening, quality evaluation, data extraction. We used Revman 5.3 software for data analysis and draw forest plot. The effect indexes were OR,MD and 95% CI.Funnel plot evaluation publication bias. Results Totally 406 studies were searched and 59 articles were preliminary chosen. After the full text reading,18 articles were eventually included. There were 5315 participants of studies were included and 2017 case groups and 3298 control groups. In the 5 studies, the results show that the incidence of osteoporosis is higher in periodontitis group than nonperiodontitis group(OR=1.71,95%CI[1.31,2.23],P<0.0001).The results of 8 studies indicated that the incidence of periodontitis is higher in osteoporosis group than nonosteoporosis group(OR=1.81,95%CI[1.45,2.27],P<0.00001).There was a statistically significant difference in the number of remaining teeth between osteoporosis group and non-osteoporosis(MD=-1.30,95%CI[-1.94,-0.65],P<0.0001).There was no significant difference in PD and BOP between osteoporosis group and non-osteoporosis(respecitively MD=-0.05,95%CI[-0.15,0.06],P=0.38 and MD=3.83,95%CI[-2.11,9.77],P=0.21).The results indicated that osteoporosis group had a significantly greater CAL,GR,ABL when compared with the non-osteoporosis group(respecitively MD=0.29,95%CI[0.12,0.46],P=0.001,MD=0.46,95%CI[0.20,0.71],P=0.0004 and MD=0.18,95%CI[0.06,0.29],P=0.003).Compared with non-osteoporosis group, osteoporosis group has a significantly significant difference in menopause duration(MD=3.28,95%CI[2.43,4.12],P<0.00001).Conclusions 1 Osteoporosis is a risk factor for periodontitis.Periodontitis is a risk factor for osteoporosis.2 Compared with the non-osteoporosis group, the osteoporosis group had a significantly greater teeth loss, alveolar bone loss, clinical attachment loss, gingival recession, and not related to periodontal probing depth, bleeding on probing.Osteoporosis group menopause duration was greater than non-osteoporosis group.
