Subject This study aims to study the anxiety and depression of chronic urticaria patients(CU),to explore the factors that may affect occurrence and development;to provide clues for the clinical identification of CU patients with anxiety or depression.The study observed the compliance compliance of patients with CU,explore effects of anxiety and depression on their medication compliance,causing further attention of clinical workers on anxiety and depression in patients with CU.Methods A total of 138 patients with CU in the Dermatology Department of Jiang Su University Affiliated Hospital were enrolled from February 2015 to December 2016.Their general condition and detailed clinical information were collected and autologous serum skin test(ASAS)and allergen test were done.The disease activities were assessed according to the urticaria activity score(UAS).According to the inclusion criteria,the patients were screened,after informed consent included in the survey.The general situation and relevant clinical information were recorded.Under the guidance of the investigators,the patients independently filled with the "urticaria activity score"(UAS)to assess the disease activity,"Hospital anxiety,depression scale "(HADS)to assess anxiety,depression.Two weeks later,patients receiving standard treatment were followed for a "Morisky medication compliance questionnaire"(MAQ-8)to assess medication compliance.After the investigation,all the data were compiled by Excel 17.0 software.SPSS 22.0 statistical software was used to analyze the data.The incidence of anxiety and depression in patients with CU was described and compared with China Normative comparison;according to the general situation,related clinical information and laboratory results CU patients were grouped and compared to explore risk factors;analysis whether there is a difference in patientswith anxiety and depression or not on medication compliance on.The corresponding data were analyzed by t test,chi square test,rank sum test,two logistic regression analysis and so on,with P < 0.05 as a significant statistical significance.Results There were significant differences in the mean scores and positive rates of HADS[a] between the groups when grouped by gender,age,birth history,education level,duration,life events,angioedema(p <0.05).The logistic regression analysis showed that gender,duration and angioedema were independent risk factors that affected the occurrence of anxiety(P <0.05).There were significant differences in the mean scores and positive rates of HADS[d] between the groups when grouped by gender,age,birth history,life events and angioedema(p <0.05).The logistic regression analysis showed that gender and angioedema were independent risk factors that affected the occurrence of anxiety(P <0.05).The MAQ-8 score in two weeks later visit showed that the CU patients were not well compliant in general,patients with anxiety and depres sion suggested by HADS had a significantly lower MAQ-8 score and compliance rating compared with those who were not and the differences were statistically significant(p <0.05).Conclusions Female,young people,experience of life events in nearly one year,with angioedema symptoms were risk factors of both anxiety and depression.In addition,unmarried,no fertility,higher education population and experience of life events in nearly one year were the risk factors of anxiety;fertility was a high risk of depression factor.In the clinical work physicians should pay more attentions in disease knowledge education and life guidance for these CU patients to reduce the psychological pressure of patients.Recommend the use of HADS in anxiety and depression screening,timely psychological counseling is needed. |
