Epidemiological Investigation And Analysis Of Health Status Of Perimenopausal Women In Gansu Province

Objective:(1)The data of baseline health status of perimenopausal women in Gansu Province were obtained.(2)To explore the related factors of perimenopausal syndrome.(3)To know the prevalence and use of menopause hormone therapy(Menopause Hormone Therapy,MHT)in perimenopausal women in Gansu Province.(4)To provide scientific basis for the formulation of relevant policies,strengthen the health education and intervention of perimenopausal women.Methods: A questionnaire survey was conducted on nearly 8500 menopausal women in 54 districts / counties and 162 townships / towns in Gansu Province by multi-level,group and random method from March to November in 2016.The questionnaire includes: 1.The basic information table,2.improved Kupperman rating scale,3.know and use MHT table.Epidata3.1 database data were used to establish the baseline data of perimenopausal women's health status in Gansu Province.SPSS18.0 statistical software was used to analyze the results.Results:(1)In total 8500 women were recruited with 8005 completed the questionnaires interview in the study,the respond rate was 92.1%.(2)Health status of perimenopausal women in Gansu Province: The average age of menopause 47.42 ±4.03 years,compared with other provinces in advance,the prevalence of perimenopausal syndrome was 78.11%.The top 5 common symptoms were Fatigue(69.02%),dizziness(62.94%),joint pain myalgia(60.62%),headache(59.19%),tension anxiety(58.12%).Modified Kupperman score overall average(15.18 ± 10.03)points.(3)Age,ethnicity,BMI,nature of work,number of births,educational level,economic status,menstrual status,regional distribution affect the occurrence of perimenopausal syndrome(P<0.05).Logistic regression analysis showed that: Age,waist-to-hip ratio,number of pregnancies,menopause were risk factors for perimenopausal syndrome(P<0.05,OR> 1);Cultural level,the nature of work,sports were protective factors for perimenopausal syndrome(P<0.05,OR< 1).(4)In Gansu Province,perimenopausal women's MHT related knowledge awareness rate of 18.25%,MHT usage rate of 0.6%.Due to perimenopausal symptoms treatment rate of15.43%,of which 37.47% of women choose obstetrics and gynecology,the main reason for menstrual disorders,hot flashes sweating,bone pain.Conclusion:(1)Menopause age ahead of time and the incidence of perimenopausal syndrome in Gansu Province is higher,the impact of perimenopausal women's physical and mental health is seriously improved.The medical service system should be improved and effective preventive measures should be taken to strengthen and promote the protection factors ? reduce and avoid the impact of risk factors on perimenopausal syndrome,to reduce peri-menopausal recent and long-term complications.(2)Gansu Province menopausal women MHT and perimenopausal self-care knowledge is weak,MHT use rate is very low.We should be based on community health education,take diversification of forms and increase the publicity of various media,to improve the level of public health knowledge and self-care awareness.(3)Improve the government and the community full of menopausal women's attention.Relevant policies and social security systems were drawed up to improve their quality of life.(4)It is recommended that all levels of medical institutions set up perimenopausal clinics,at the same time to strengthen the medical staff MHT education and training for the local menopausal women do health care work.
