| Background In the field of urology,the incidence rate of renal cell carcinoma ranked second,only after bladder cancer.As many malignant tumors,the pathogenesis of renal cell carcinoma has always troubled human beings worldwide.In order to improve the early diagnostic rate of renal cell carcinoma and improve patients’ prognosis,the study of renal cell carcinoma pathogenesis is becoming a hot today.In the CD4+T cells subgroup,the newly discovered Th17 and Treg cells play an important role in the pathogenesis of renal cell carcinoma.In this paper,we will study spreadly in order to research what kind of role Th17 cells and Treg cells played in the development of renal cell carcinoma,and also what kind of inextricable link between these two cells?Purpose To study the expression levels of Th17 cells and Treg cells in peripheral blood of patients with renal cell carcinoma,to explore whether the expression levels of these two cells in renal cell carcinoma patients and the normal population is different,and whether there is a link between the pathological changes of renal cell carcinoma,at the same time to explore whether there is a balance between Th17 cells and Treg cells in the peripheral blood in patients with renal cell carcinoma.Materials and Methods1.According to the preoperative history and clinical examination,collecting 49 patients with primary renal cell carcinoma that was first hospitalization in Urology second department of Lanzhou University Second Hospital from January to August,2016.Pumping preoperative fasting peripheral venous blood of these patients 5 ml after the consent of the hospital,the patients and their family,of which 3 cases were not treated surgically,without pathological guarantee,the experimental results were discarded;Of 1 case postoperative pathology diagnosis was leiomyoma,the experimental results were discarded;Of 1 case postoperative pathological diagnosis was malignant fibrous histiocytoma,the experimental results were discarded.2.The remaining 44 cases including: 35 cases of clear cell carcinoma.5 cases of papillary cell carcinoma.3 cases of chromophobe cell carcinoma.1 cases of uncategorized renal cell carcinoma.According to the criteria of the 2010 AJCC renal staging and staging combination,we divided these cases into I.II.III.IV.All patients were not treated with any anti-tumor and immunotherapy before surgery.10 cases of normal control group from our college graduate students and physical examination of normal people of physical examination center,the crowd were willing to accept the test.3.The expression levels of Th17 and Treg cells in peripheral blood of patients with renal cell carcinoma and the normal control were measured by flow cytometry,so we can explore the significance of Th17 cells,Treg cells and Treg/Th17 levels in normal control group and each stage of renal cell carcinoma.At the same time,the general clinical data of each study object were analyzed,such as sex.age.pathologic type.urea,creatinine.urea creatinine ratio and so on,to explore the relationship between the expression levels of Th17 and Treg cells in peripheral blood of patients with renal cell carcinoma and the clinical indicators.4.The experimental data were analyzed by statistical software SPSS19.0.Results1.Compared with the normal control group,the expression level of Th17 and Treg cells in peripheral blood of patients with renal cell carcinoma was significantly increased(6.195% ± 2.267% vs 2.892% ± 1.886%,7.797% ± 3.005% VS 4.764% ± 0.435%),the differences were statistically significant(p<0.001);While the Treg/Th17 ratio was slightly decreased(1.351% ± 0.497% VS 2.157% ± 1.133%),in statistics,the difference do not make sense(p> 0.05).2.In patients with renal cell carcinoma,because there is only one case of patients with stage Ⅳ,the experimental results are not an independent stage to participate in the comparison,in the remaining three phases,with the increasing of TNM staging,the expression level of Th17 cells and Treg cells increased gradually,While the Treg/Th17 ratio is no significant increase or decrease(Th17 cells: 5.281% ± 1.881% <7.074% ± 1.400% <9.208% ± 2.077%,< <Ⅰ Ⅱ Ⅲ;Treg cells: 6.176% ± 0.996% <9.248% ± 2.390% <12.435% ± 2.869,Ⅰ< Ⅱ<Ⅲ;Treg/Th17: 1.326% ± 0.557%> 1.305% ± 0.185% <1.413% ± 0.484%,> < Ⅰ Ⅱ Ⅲ),Among them,Th17 cells and Treg cells were statistically significant compared with each other(p <0.05);But there was no significant difference in Treg/Th17 ratio(p> 0.05).With the increase of renal cell carcinoma staging,Th17 cells and Treg cells showed a decreasing trend gradually,Th17 cells and Treg cells showed a positive linear relationship(r= 0.5806,p <0.0001).3.We classified stageⅠ,stage renⅡ al cell carcinoma as early renal cell carcinoma,stage Ⅲ,stage renal cell carcinoma as advanced renal cell carcinoma,compared with early renal cell Ⅳcarcinoma,the level of Th17 cells and Treg cells in advanced renal cell carcinoma was significantly increased,the differences were statistically significant(p<0.01).Treg/Th17 ratio was slightly increased,in statistics,the difference do not make sense(p> 0.05).4.Different pathologic types of renal cell carcinoma were compared with each other we found the differences of Th17.Treg.Treg / Th17 ratio of different pathologic types were not statistically significant(p> 0.05).Conclusion1.Th17 cells and Treg cells in peripheral blood are closely related to the development of renal cell carcinoma,besides,in the body’s immune system,the increase of Th17 cells and Treg cells may promote the development of renal cell carcinoma,but there is no association compared with the heterogeneity of differentiation of renal cell carcinoma original cells.2.Besides,the extent of its increase was consistent with the pathological stage of renal cell carcinoma,the higher the degree was,the higher the pathological stage became,in other words,Th17 cells and Treg cells are also closely related to local progression and distant metastasis of renal cell carcinoma.3.Moreover,there was a dynamic balance between Th17 cells and Treg cells in normal patients,and there was a significant imbalance between Th17 cells and Treg cells in peripheral blood of patients with RCC,with the development of medical technology,which may bring some new ideas and methods to us in the diagnosis of renal cell carcinoma,4.Reducing the proportion of Th17 cells and Treg cells in patients with renal cell carcinoma,reshaping the dynamic balance of Th17 cells and Treg cells,may provide a new immunological target and help for the treatment of renal cell carcinoma and improvement of the prognosis. |
