| Objective: The diagnosis of hematological diseases should be based on test results of the laboratory as an objective basis.The accuracy of the results directly affects the diagnosis,treatment and rehabilitation.With the advancement of technology,the proportion of measurement error is getting smaller and smaller.The literature shows that the accuracy of the results is closely related to the quality management of the test.Therefore,quality management is one of the core works of blood disease laboratory.The aim of this study is to establish an evaluation method for the quality management of hematological diseases in the Chongqing area.And thus a more effective quality control management would be set up and the validity will be verified in a relevant hospitals.Such it may improve the level of laboratory diagnosis and testing for better clinical services.Moreover,it would provide a basis for the purpose that quality control of blood disease laboratory of Chongqing close to the level of the international advanced.Methods: Through the literature collection and reading,blood disease quality control of laboratory reports related to progress were screened.Summarized the results and experience of quality control in recent years and combined the literature research,the theory connotation and extension of quality control,dynamic,information quality management was elaborated.And provide theoretical support for hematological laboratory testing quality control.Blood laboratory quality control theory would be finally formed.Based on those,60 representative hospitals in Chongqing were selected to design a questionnaire on the basic situation of blood diseases laboratory.The main topics included establishment of quality management system,staffing and training,equipment and equipment management,quality control development,and so on.To solve these problems,a quality management plan was further developed.And the preliminary verification application of the quality control plan is to be carried out in the Xinqiao Hospital Hematology Laboratory to test the effectiveness of the management methods proposed in this study.Results: Theoretical research has enriched the connotation of quality control and the concept of information quality control was understood.Moreover,the content of real-time quality control was further studied.According to the return of 57 valid questionnaires,the survey analysis found that only 14% of hematological specialist laboratories were specialist.However,most of those can complete the basic blood test items.Of which 66.6% have a complete experimental process and SOP files,Personnel to intermediate grade and bachelor degree.35.1% of the experimental equipment more than 5 million Yuan,45.6% of the inspection record.About 45% of the inspection report will refer to the clinical manifestations,and strict examination.All the laboratories carried out the routine indoor quality control well,and participated in the indoor quality evaluation of the city;but obviously,the room quality control out of control participation frequency is less than the indoor quality control.In addition,the survey results also found some problems,such as backward equipment,lack of personnel,a serious lack of remote consultation system,learning less training,sample collection failure rate,LIS hardware and software compatibility issues.According to the survey results,quality management improvement programs were developed,such as the development of "probability of occurrence of the problem matrix",setting up "sample collection tube circle." The data showed that the sample collection defect rate decreased from 3.66 ‰ to 1.05 ‰;the preparation process of the specimen was greatly optimized,and the qualified rate of sample preparation was obviously increased.The detection cycle was accelerated,for instance,chromosome and flow detection time shortened by 2 days,other projects shortened 0.5 to 1 day range;In addition,the clinical compliance rate has also been significantly improved.These results show that the results of "quality management improvement program" to verify achieved good effect.Conclusions:Based on the literature review and extensive investigation,the preliminary improvement measures of quality management in hematological laboratory were established.The verification results showed that the improved measures enhanced the qualified rate of the samples,shortened the testing period and heightened the clinical coincidence rate.The set of improvement measures can significantly enhance the quality of blood laboratory management,in order to improve blood quality in Chongqing blood laboratory management and provide a solid theoretical and experimental support. |
