| Objective: To explore the factors that affect the long-term survival of patients with HCC after partial hepatectomy,so as to provide a theoretical basis for the clinical treatment and prognosis evaluation of patients with HCC.Methods: The information of patients with HCC who had been accepted hepatectomy and survived more than 5 years was analyzed retrospectively.Select some factors that may affect the prognosis of patients with HCC after hepatectomy,such as age of the patients,gender,number of tumors,tumor size,preoperative Child-Pugh Score,preoperative ALT,AST,TBIL,GGT,ALB,AFP,perioperative blood transfusion,tumor recurrence and treatments after recurrence.The method for calculating and analysizing was the software of SPSS 22.0.The survival rates and disease-free survival rates were calculated with the method of Kaplan-Meier.The different factors that related the prognosis of HCC were screened by the Log-rank test and Cox regression.Results: Univariate analysis showed that the number of tumors,BCLC and perioperative blood transfusion were the risk factors of tumor recurrence in patients with longer survival(P <0.05).Multivariate analysis showed that the number of tumors and perioperative blood transfusion were independentinfluencing factors for tumor recurrence in patients with longer survival time.Univariate analysis showed that gender,age,number of tumors,BCLC,perioperative blood transfusion,and treatments after recurrence were associated with the prognostic of patients with longer survival time(P <0.05).Of all the patients included in the study,101(42.6%)had tumor recurrence.The Log-rank test according to the treatments after recurrence showed that the median survival time of patients who accept reoperation was significantly longer than that of non-surgical patients(P <0.05).COX multivariate regression analysis showed that age,number of tumors and perioperative blood transfusion were independent prognostic factors of patients with longer survival time(P <0.05).Conclusions: 1.gender,age,number of tumors,BCLC,perioperative blood transfusion,and treatments after recurrence are associated with the prognostic of patients who survived more than 5 years after surgery(P <0.05).And age,number of tumors,perioperative blood transfusion are independent prognostic factors.2.For patients who survived more than 5 years after surgery,when the tumor recurrence re-surgical treatment is significantly better than non-surgical treatment. |
