Clinical Significance Of Dickkopf-1 On Disease Activity,Bone And Joint Damage In Patients With Rheumatoid Arthritis

Objective: We examine the serum and synovial fluid levels of DKK-1 in patients with RA and investigate their relationship with disease activity,bone and joint damage in RA.Methods: Serum levels of DKK-1 in 35 patients with RA,27 patients with OA and 25 healthy controls were detected by enzyme-linked immunosorbent assay(ELISA).At the same time,paird synovial fluid were collected from 17 patients with RA and 10 patients with OA,using ELISA to detect synovial fluid levels of DKK-1.We kept a record of all the clinical and laboratory results simultaneously,including gender,age,duration of disease,BMI,the number of tender and swollen joints,ESR,CRP,VAS scores,RF,ACPA,DAS28 scores.Meanwhile,radiographs of two hands in all patients with RA were evaluated according to the Sharp’s method.Results: 1.The serum DKK-1 levels in RA group are higher than that of OA group(4251.45±3139.04 pg/ml vs 2700.15±1793.04 pg/ml,P=0.024),which is of statistical significance(P<0.05).The serum DKK-1 levels in RA group are higher than that of healthy controls(4251.45±3139.04 vs2875.76±2522.47 pg /ml,P=0.055),but there is no statistical significance.2.The serum levels of DKK-1 are apparently higher in active RA patients than that of inactive RA patients(6025.31±3721.20pg/ml vs 2731.63±943.59pg/ml,P=0.005),which is of statistical significance.3.There is no statistical difference in the serum levels of DKK-1 among inactive RA patients,OA patients and healthy controls(2731.63±943.59 vs 2700.15±1793.04 vs2875.76±2522.47 pg/ml,P=0.971,P=0.828).4.The synovial fluid levels ofDKK-1 in RA group are higher than that of OA group(113.59±112.55pg/ml vs61.60±40.67pg/ml,P=0.106),but there is no statistical significance.5.The serum levels of DKK-1 in RA patients are obviously increased than that of paird synovial fluid(5464.89±3849.50pg/ml vs 113.59±112.55pg/ml),which is of statistical significance(P<0.001).There is no correlation between serum and paird synovial fluid DKK-1 levels in 17 patients with RA(r=0.428,P=0.087).6.The serum DKK-1 levels in RA group are related to the number of the tender joints,the number of swollen joints,ESR,CRP,RF,DAS28 scores,and relevant coefficients are respectively0.474,0.566,0.739,0.538,0.424,0.596(P<0.05);on the other hand,it has no relation to age,duration of disease,BMI,ACPA and VAS scores.7.The serum DKK-1 levels in RA patients are positively correlated with joint narrowing space scores,joint erosion scores and Sharp scores in radiographs of hands with RA,and relevant coefficients are 0.416,0.454,0.409 respectively(P<0.05).Conclusion: 1.Serum DKK-1 levels in RA group are significantly higher than that of OA group,which is statistically significant,and have an increasing trend than that of healthy controls;the serum levels of DKK-1 are apparently higher in active RA patients than that of inactive RA patients,OA patients and healthy controls,while there is no difference among inactive RA patients,OA patients and healthy controls,which indicate that DKK-1 may be served as a biomarker in RA patients and may be related to inflammation in RA.2.The serum levels of DKK-1 in RA patients are obviously increased than that of paird synovial fluid,which is of statistical significance.The synovial fluid levels of DKK-1 in RA group have an increasing trend than that of OA group.There is no correlation between serum and paird synovial fluid DKK-1 levels in patients with RA,which suggest that expression of DKK-1 inserum and synovial fluid of RA may be relatively independent.3.The serum DKK-1 levels in RA group are related to the number of the tender and swollen joints,ESR,CRP,DAS28 scores,it has no relation to age,duration of disease,BMI,ACPA and VAS scores.,which suggest that DKK-1 is closely related to disease activity in RA.4.The serum DKK-1 levels in RA patients are positively correlated with joint narrowing space scores,joint erosion scores and Sharp scores in RA,which indicate that DKK-1 is associated with bone erosion in RA.
