| Objective:Through the study,clinical curative effect on elderly patients with coronary heart disease with chronic heart failure to observe the Fuling Wenshen capsule combined with conventional western medicine and the safety of blood vessels in the serum of patients with angiotensin II,aldosterone,effects of arginine vasopressin and N-terminal pro brain natriuretic peptide.To further explore the mechanism of the Fuling Wenshen capsule in the treatment of chronic heart failure in elderly patients with coronary heart disease.Method: 1.According to the diagnostic criteria of TCM and Western Medicine on chronic heart failure of coronary heart disease heart failure diagnostic criteria,diagnostic criteria,NYHA cardiac functional grading standards to develop a unified standard,screened 60 CHF patients met the inclusion criteria were randomly divided into study group and control group of 30 cases.2.The control group was given conventional western medicine treatment,30 patients in the study group were treated with routine western medicine treatment on the basis of the use of Poria capsule,the treatment of the two groups were all for 4 weeks.3.To evaluate the curative effect and heart function in patients with heart failure,Lee’s integral medicine ultrasound before and after treatment(LVESD,LVEDD,LVEF)and TCM symptoms integral to evaluate cardiac function.The serum Ang II,ALD,ADH and NT-pro BNP were detected before and after treatment.4.All the data were analyzed by statistical software SPSS 19.0,and the measurement data were expressed by "-x ±s ".The paired data “t” test was used to compare the differences between the two groups before and after treatment in the group,and the test of the independent sample “t”.Count data using chi square test.Rank sum test.The difference of P<0.05 was statistically significant,P<0.01 was statistically significant.Result: The study group and the control group after treatment: after treatment,the two groups of NYHA cardiac function classification efficacy evaluation were improved,the study group was better(P<0.05).The two groups of LVESD,LVEDD and LVEF were improved after the treatment,and the curative effect was better in the study group(P<0.01).Two groups of Lee’s heart failure scores were decreased,the study group decreased significantly(P<0.05).The scores of symptoms and signs in two groups were decreased,the clinical symptoms were relieved,and the study group was more obvious(P<0.05).Two groups of serum Ang II,ALD,ADH and NT-pro BNP content decreased,the study group was better(P<0.05).There was no adverse reaction in the two groups.Conclusion:In the treatment of senile chronic heart failure of coronary heart disease in the routine western medicine combined with Fuling Wenshen Capsule can obviously improve the heart function of patients,improve the quality of life,reduce hospitalization rate,improve prognosis,and the curative effect is better than western medicine group.The mechanism may have a few points:(1)Fuling Wenshen Capsule can act on the renin angiotensin aldosterone system,inhibit the production of Ang II,reduce circulation and local Ang II levels,thereby inhibiting myocardial remodeling,delaying the process of heart failure.(2)Inhibition of ALD formation,improvement of cardiac endothelial function,improvement of myocardial ischemia,reversal of myocardial lesion,prevention and delay of cardiac remodeling.(3)In addition to reduce the generation of ADH,reduce sodium and water retention,reduce heart burden,improve cardiac function. |
