| Objective: To observe the clinical efficacy and safety of Tckahoe wenshen in the treatment of elderly patients with coronary heart disease with chronic heart failure; To observe the changes of serum UA and IL-6, HCY, NT-pro BNP and Tckahoe wenshen before and after treatment; to further explore the mechanism of Tckahoe wenshen intervention in elderly coronary heart disease and chronic heart failure.Methods:1. According to the diagnostic standard of coronary heart disease and chronic heart failure diagnostic criteria, NYHA cardiac function classification standard and Chinese medicine diagnostic criteria formulated were consistent with the inclusion criteria,collected in line with 60 cases of patients with traditional Chinese medicine and Western medicine diagnosis, were randomly divided into treatment group and control group with30 cases in each.2.The treatment group was treated with Tckahoe wenshen plus conventional western medicine, while the control group was treated with routine western medicine only. 4weeks for a course of treatment.3.Before and after treatment observed in patients with heart function to evaluate the efficacy of traditional Chinese medicine and Western medicine Lee’s failure integral,echocardiography(shoot blood fraction(LVEF)(%) and TCM syndrome integral to be assessed in patients with heart function(NYHA). Before and after treatment, extraction of serum UA, IL-6, changes of homocysteine(Hcy) and NT pro BNP for laboratory testing and data recording.4. Collect all the data and make the case data form. All data using SPSS statistical analysis: "-x ±s " representation of data, were compared before and after treatment with paired t-test, before and after treatment between the two groups compared with the independent samples t test, P < 0.05 difference was statistically significant, P < 0.01 difference has statistical significance.Results:1. Treatment group before treatment and after treatment contrast: treatment after cardiac function in patients with clinical evaluation of traditional Chinese medicine and Western medicine in Lee’s failure integral, echocardiography(shoot blood fraction(LVEF)(%)and TCM syndrome score were improved; serum uric acid(UA), IL-6, Hcy, NT pro BNP levels numerical decreased significantly, was statistically significant(P < 0.01).2.The control group before treatment and after treatment contrast: treatment after cardiac function in patients with clinical evaluation of traditional Chinese medicine and Western medicine in Lee’s failure integral, echocardiography(shoot ejection fractions(LVEF(%) and symptom scores were improved; serum UA, IL-6, changes of homocysteine(Hcy) and NT pro BNP value decreased significantly, with statistical significance(P < 0.01).3. In the treatment group, control treatment group after the numerical comparison: after treatment, the treatment group NYHA class is better than that of the control group(P <0.05); western medicine Lee’s heart failure score, TCM syndrome integral were improved, treatment group significantly(P < 0.01); LVEF values in the two groups were all improved, curative effect of treatment group was better(P < 0.05); two groups of serum UA, IL-6 and Hcy in the changes of NT pro BNP value decreased, the treatment group decreased significantly more(P < 0.05); two groups did not appear serious adverse reactions.Conclusion: Tckahoe wenshen combined with conventional western medicine in the treatment of Yang deficiency of heart and kidney failure, its curative effect than western medicine, can significantly improve the life quality of patients with cardiac function,and its mechanism may be: 1 in RAAS system, reduce the sodium and water retention,reduce the load on the heart, improve heart function; 2 by inhibiting the degradation of collagen and matrix remodeling, thus the improvement of myocardial remodeling and delay the process of CHF; 3 by inhibiting the inflammatory reaction, abnormal expression of inflammatory cytokines, reduce myocardial injury, thereby improving cardiac function; 4 by adjustment of autonomic nervous system in patients with Yang deficiency of heart and kidney function failure, reduce sympathetic activity, excited vagus nerve, play the role of anti myocardial ischemia and hypoxia and improve heart function. |
