Objective:This paper analyzes the risk factors of medical disputes in a third-grade class-B hospital in Changchun City by means of archives and questionnaire.following the research ideas from the phenomenon to the essence,we puts forward the countermeasures to to deal with the medical dispute for the hospital to establish a harmonious and healthy relationship between doctors and patients,and provide a reference for maintaining the normal order of medical services.Methods:First of all,this study used a number of databases to review the literature for understanding the current situation of medical disputes,influencing factors,research methods and solutions.Through the hospital from January 2014 to January 2016 occurred during the medical dispute cases collated and analyzed one by one,this study drew the status of the hospital disputes.Combining with the literature,and referencing to a number of existing research used in the questionnaire,consultants and medical staff,we developed a questionnaire in line with this subject.Then,the study conducted a pre-survey,we modified the questionnaire by credit validity analysis.Finally,we conduct a formal questionnaire survey of the relevant medical staff.Data were recorded and sorted by EXCEL software.The data were analyzed by SPSS 18.0 software.The counting data were described by rate and composition ratio.The chi-square test and rank-sum test were used for statistical analysis,with α = 0 05 as the test level to analysis of the causes of medical disputes and the impact of factors.Ultimately,we made constructive suggestions for the hospital to solve the problem of medical disputes.Results:(1)During the period from January 2014 to January 2016,82 medical disputes occurred in this hospital,which with no major or malignant medical dispute.The reasons for the disputes included medical technology,service and treatment costs,the proportion of the three were 36.59%,26.82% and 36.59%.Which belong to the medical misunderstanding of the total number of disputes accounted for 76.83% of the number of cases.Gynecology and pediatrics are higher incidence of medical disputes departments,respectively,0.28 ‰ and 0.26 ‰,much higher than that in the same period of other departments.(2)Among involved medical personnel,the overall proportion of men than women,aged 40 to 49 years of age and working years 15 to 24 years of medical staff in the proportion of the largest number of colleagues,master’s degree or above is also a high incidence of medical disputes.(3)The survey found that there were statistical differences in the degree of occupational perception such as tension,physical response to work intensity and overall satisfaction.In the knowledge dimension,the degree of medical dispute risk familiarity,the doctor’s duty and the patient’s right to understand the degree of medical dispute risk Training attitude had statistical differences;in the attitude and behavior dimensions,the doctor-patient relationship,the occurrence of medical disputes when the primary treatment,medical risk attention,to introduce the patient’s condition,care about the patient’s feelings,the cost of communication had statistical differences.Conclusion:(1)The main reasons for the hospital disputes,including medical technology reasons,service reasons and treatment costs reasons,but most of them belong to medical misunderstanding.(2)The factors affecting the medical staff of the hospital medical treatment included as follow : gender,age,working life,education,stress in the work,physical strength,overall satisfaction,familiarity with medical dispute risk,doctor’s responsibility and patient rights,the attitude to medical dispute risk training,the attitude to the doctor-patient relationship,the occurrence of medical disputes when the primary treatment,medical risk attention,whether be inclined to introduct the patient’s condition,whether care about the patient experience,and whether be inclined to communicate the cost.(3)According to the existing problems and influencing factors,the hospital should strengthen the education and training of doctors and patients to communicate,improve the quality of medical care,improve the satisfaction of medical personnel,strengthen the management of medical expenses and strengthen the knowledge of medical disputes,and establish the medical dispute crisis management system.
