Objective:By observing the changes of sexual function and relevant indicators after using aromatase inhibitor(letrozole),to investigate the efficacy and safety of letrozole in treatment of erectile dysfunction(ED)with hyperestrogenemia,thus providing a new therapy and guidance for the clinical treatmen of patients with ED related to high level estrogen.Methods : A total of patiens with ED and high estrogen included from 2015.11 to2016.11 in the first affiliated hospital clinic of xxxx university,in which 12 patients were diagnosed with olioasthenospermia.All the eligible patients were administrated letrozole 2.5mg qd,and the therapeutic duration is 1 month.After 1 month,collecting the relevant needed datas and observing the efficacy and the side effect.If there were no sever complications,patients can be orally administrated with letrozole until 3month.In the duration of therapy,patients were informed to stop using other drugs related to male sexual function and serum sex hormone levels.After 1 month and 3months,some symptoms and indicators were observed including the serum sex hormone levels(estrogen,testosterone,FSH,LH),semen parameters(semen volume,semen concentration,sperm count and sperm motility),changings of erectile function assessed by IIEF(Internationnal Index of Erectile Fuction),sexual desire,the testis volume and some side effects.Results:1 of 32 patients lost to follow-up,and 1 patient gave up the treatment because of being not satisfied with the efficacy of the medicine.As a result,there were 30 patients have completed this clinical trial actually.The serum estrogen level decreased significantly(p=0.021).There were significant improvement between pre-treatment end post-treatment on the indicators of the serum sex hormone levels(testosterone,FSH,LH,T/E),semen parameters(sperm count,semen concentration,sperm motility),IIEF score and frequency of sexual intercourse(p<0.05).Hoewer,there were no significant statistics difference on the indicators of semen volume,normal sperm rate and the testis volume.In the all patients,there no sever side effects except some mild complications such as decrease on sexual desire(3 cases),slight headache(2 cases),the transient riseof glutamic-oxalacetic transaminase(1 case).Conclusion:The aromatase inhibitor(letrozole)can effectively enhance the IIEF score of ED patients with high estrogen level,and improve the sexual function;as for the patients with olioasthenospermia or azoosperm,administrating of letrozole can also partly improve the semen parameters and fertility rate.Furthermore,no sever complications were found on letrozole so far.So,letrozole may can be the effective supplementary means for treating ED with hyperestrogenemia,olioasthenospermia,or infertility in the future.
