Background and aim:The effectiveness and safety of endoscopic techniques have been confirmed by various studies.As the superiority of little trauma,prompt recovery,no incisional scar compared with surgery,the technique has gained widespread acceptance.The iatrogenic ulcer induced by endoscopic techniques especially endoscopic submucosal dissection is different from traditional peptic ulcer,which has been taken more and more attention.The aim of the study is to research the possible factors which influence the healing of iatrogenic ulcer.Methods:We retrospectivly reviewed patients who underwent endoscopic submucosal dissection due to various gastric disease at the first affiliated hospital of Zhejiang University from 2012.1.1 to 2016.12.31.According to the inclusion criteria and exclusion criteria,we enrolled 104 patients finally.We analyzing the relationship between factors such as sex,age,lesion location,the size of iatrogenic ulcer,the status of helicobacter pylori.infection,basic disease such as diabetes and the dalayed iatrogenic ulcer healing,especially helicobacter pylori.infection.Result:The healing rate of iatrogenic ulcer is 68.27%(71/104)1 month after ESD and 99.04%(103/104)3 months after ESD.The 1-month healing rate of patients who were infected with HP has no difference compared with patients who were not(58.97%vs.73.85%,P=0.115).However,the 1-month healing rate of patients whoes iatrogenic ulcer's long diameter was more than 3 centimeters is much lower than patients whoes iatrogenic ulcer's long diameter was less than 3 centimeters(61.76%vs.80.56%,P=0.049).And in patients whoes iatrogenic ulcer's long diameter was more than 3 centimeters,the 1-month healing rate of patients who were infected with helicobacter pylori.is much lower than those who were not(40.91%vs.71.74%,P=0.014).Conclusion:The majority of iatrogenic ulcer could heal in 3 months after ESD.The status of HP infection has no no obvious correlation with delayed iatrogenic ulcer healing,but the long diameter more than 3 centimeters may be closely related to delayed iatrogenic ulcer healing,and is a independent risk factor.Patient whoes iatrogenic ulcer's long diameter was more than 3 centimeters may obtain benefit from HP eradication.
