Clinical Study Of The Nutrition Effect Of The Critically Ill Patients By The Nasal Intestinal Perfusion Method

Objective:To compare of the different nutritional effects of the two kinds of gastrointestinal perfusion ways,this study used the following way:the nasointestinal tube enteral nutrition and nasogastric tube enteral nutrition.The content of this study contained the nutrition indicators,gastrointestinal complications and clinical curative effect etc.In addition to choosing the most effective method for the clinical nutrition support of the critically ill patients,and reducing the stress response after severe trauma and improve the clinical efficacy.It could provide more theoretical basis for the better application of enteral nutrition in clinical intensive medicine.Methods:This study used a convenient method that selected 60 critical patients in accordance with the standard of ICU that was enrolled in a general hospital in Yanji city from December 2015 to October 2016.The patients were randomly divided into two groups in accordance with programming the hospital number.One was the observation group(n=30)(nasointestinal tube enteral nutrition group)and the other was the control group(n=30)(nasogastric tube enteral nutrition group).The nurses were investigated by the questionnaire of the nutritious knowledge before the intervention.According to the results of the questionnaire,the nurses were selected to train.They could come to the interventional experiment,if the nurses passed through these tests.The critical patients that admitted to the hospital obtained the enteral nutrition after 24 hours.To be limited to 7 days,the critical patients were exsanguinated to detect the nutrition index(the total serum protein、serum protein、hemoglobin and prealbumin).The times of the observing laboratory indicators were four stages,such as the prior treatment、the the first day、the third day and the seventh day after treatment.The nutritional risk screened table(NRS2002)was used to assess the nutritional status of patients and clinical efficacy between the treated group and the untreated group.The author analyzed whether there were the statistical significance.The baseline information of two groups was not statistically significant,such as age、gender、disease severity、GCS score、standard swallowing function evaluation scale(SSA)and APACHE-II.Results:The effect of enteral nutrition support of two groups of patients with severe neurological diseases(1)two groups compared that there were no statistically significant(P>0.05),such as age、sex、severe degree of the disease、GCS、SSA、APACHE-ⅡComparable.The relative contents:Nutrition risk screened scale(NRS2002)and the nutrition indexes(the total serum protein、Serum albumin、hemoglobin and prealbumin)were compared when they were the prior treatment that the difference was not statistically significant(P>0.05)(2)the comparison of nutritional risk screened scale was between the treated group and the untreated group,the difference was statistically significant(P<0.05).The score of nutritional risk screened scale of the prior treatment was no difference and statistically significant(P>0.05).The score of nutritional risk screened scale decreased significantly by the nasointestinal tube in the third and seventh day.It showed that the intervention group was better than the control group.(3)Their nutritional parameters were significant difference and statistically significant between the two groups when they came to the intervention experiment(P<0.05).The nutritional parameters of Hemoglobin of the first day and the third days were no difference and statistically significant(P>0.05).There were significant differences of hemoglobin between the two groups after intervention for seventh days(P<0.05).It showed that the intervention group was better than the control group.(4)The complication rate of the intervention group was significantly lower than its of the control group,the two groups had significant differences and statistical significance(P<0.05).Conclusion:Comparison of two kinds of the enteral nutrition(1)The nutritional risk screened scale of the score in the intervention group was more reduced,because the nutritional intervention time was more prolonged.The incidence of malnutrition in critically ill patients was decreased and the nutritional status was improved.The nutritional risk screened scale of the score in the control group was slowly increased,the incidence of malnutrition of the patients was increased in this group.(2)The nutritional parameters of severe patients would be improved by the nasointestinal tube nutrition,the nutritional parameters of the nasogastric tube nutrition declined slowly.Furthermore,it showed that the intervention group was better than the control group.(3)On incidence of complications:it showed that the intervention group was better than the control group,such as gastric retention、reflux、aspiration.high blood glucose.(4)The nurses need strengthen the cognition of the method of the tube perfusion,and mast the relevant concept and the practice,in that they do not know the relevant nutritional knowledge.The nurses must master the comprehensive nutrition knowledge to improve the nurse’s skill.
