Objective To explore and discuss the pathophysiological mechanisms of post concussion syndrome,via using the techniques of resting-state functional magnetic resonance image and build the brain structural network model:(1)To find the microstructural damage and discuss the clinical significance by building the white matter fiber network of patients with mild traumatic brain injury using diffusion tensor magnetic resonance imaging.(2)To find the gray matter volume changes followed mild traumatic brain injury and discuss the clinical significance using voxel based morphometry f MRI and computer-aided graphics processing technology.Method(1)Recruiting and selected 18 PCS patients from neurosurgery clinic of Anhui provincial hospital during the period of April 2015 and September 2015,with the same time 18 sex,age,education-matched healthy adults were recruited making up the control group,all subjects were taken f MRI scan at the third month after injury,all data were disposed successively eddy correct,tensor calculation,spatial normalization,and after calculated the fractional anisotropy(FA)values and randomization statistical comparison with control group,we intended to investigate the damage of white matter.(2)23 PCS patients and 25 healthy persons making up the patient group and the control group,after obtained the rs-f MRI data,use the VBM software to process the data,including standardization of image transformation,Registration,image segmentation,smoothing and statistic analysis,we analyzed the changes of gray matter volume and investigated the damage of cortex,extract significant results and visually display.Results(1)It revealed in the Johns Hopkins University White Matter template that:compared with the normal group,the patients group with FA value regions mainly in:corpus callosum knee and body,bilateral corona radiata,the left posterior thalamic radiation(including radiation beam)and right external capsule;increased FA value regions were not found.(2)Based on the VBM method,the results showed that:decreased gray matter volume were found in areas including left inferior frontal gyrus,left superior temporal gyrus,right rectus,left operculum insulae and left side of the putamen.While increased gray matter volume were not observed.Conclusion(1)The microstructural damage of white matter fibers were found in patients with mild traumatic brain injury,which may be a link with clinical syndrome after concussion.(2)Gray matter volume changes were found in patients with mild traumatic brain injury by VBM-MRI,which may be a link with post-concussion syndromme.In conclusion,this study adopts the resting-state f MRI brain imaging technology,to be able to show that the brain white matter and gray matter network alteration occurred in patients with post concussion syndrome,as for the pathogenic mechanism of research,this study may provide a new direction,and for the diagnosis and treatment of traumatic diseases which may provides a new theoretical basis.
