The Enlightenment To The Construction Of Hospital Management Mechanism On The Cognitive Status Of The Relationship Between Doctors And Patients

Objective:To understand and subjective evaluation of Jiangsu Province medical media three party for the doctor-patient relationship since the understanding of the new medical reform;to provide feasible suggestions for the reform of public hospitals and the establishment of a modern hospital management system;the analysis of the factors affecting the doctor-patient relationship and harmonious doctor-patient roots,and explores the reasons of media three party cognitive differences,differences in patient three media the party made system analysis and evaluation approach and emphasis given internal management and supervision of government hospital from the management perspective,to provide reference for the construction of a harmonious doctor-patient relationship.Methods:Based on literature research,questionnaire survey and interview,the author analyzed the content of the research by the use of the framework analysis and the related statistical methods.Professional journals and documents related to government health and related information on the network since the new medical reform,query research,access to relevant literature at home and abroad in the database;questionnaire survey in Nanjing,Wuxi,Nantong,Huaian four City,selected four comprehensive hospitals;according to the status of doctor-patient relationship and its influence factors of medical media three party awareness,take individual interviews and expert interviews with a combination of form,in the views of the construction scheme of harmonious doctor-patient relationship;through the analysis of main framework analysis framework of the original data construction method,interview and literature data analysis after the integration process,finally obtained the main hospital management to improve the doctor-patient relationship based on the framework.According to the framework design of the questionnaire;the use of Epidata3.0 double entry of data,the data using office software After finishing the preliminary into SPSS20 for descriptive and inferential statistical analysis for comparison of the adoption rate of chi square test for the doctor-patient relationship and influence factors on policy recommendations using the Logistic regression method.Results:40.6%patients and 41.7 members of the media on the doctor-patient relationship in the evaluation of more than 80,much higher than the proportion of medical personnel(9.2%);the top three departments that the doctor-patient relationship is very poor,31.1%,Chinese 25%,23.9%surgical survey evaluation of doctor-patient relationship by practicing environment,individual differences,social environment;high medical cost is hindering factors influencing the harmonious doctor-patient relationship as one of the top five only one common factor of the three party medical media cognition,which doctors think is the top three media(82.8%),improper defects of government health care reform(60.3%),social difference(58.5%),patient that is the high cost of medical treatment(56.7%),(49.6%)the impact of market economy,poor attitude of the medical staff(28.1%),members of the media think are high medical costs(60.7%),(57.7%)the impact of market economy,medical Staff service attitude(53.4%);more than 70%patient thinks that hospital facilities and improve the environment,improve service attitude,service process has been optimized;the three party media doctor-patient think is very important for individuals to improve the doctor-patient relationship were more than 50%,of which the highest proportion of hospital patients(76.8%),second(59.8%)the media,for 55.2%;93.2%of the medical staff,80.1%patients and 73.6%media that are willing to do their efforts to improve the doctor-patient relationship;that need to punish the violent injury medicine doctor and patient and the media were respectively accounted for 94.7%,73.3%and 81.6%groups;three party media policy of the new medical patient awareness is low,understand and understand accounted for 48.2%,21.1%and 11%;the importance of doctor-patient communication to build a harmonious doctor-patient relationship by 85.30%media personnel,91%of the patients And 91.50%of the doctors must;doctors work busy and the conflicts of interests between doctors and patients is mainly caused by the three party medical media that the doctor-patient communication.Conclusions:The relationship between doctors and patients is not socially sensitive,and the development trend of doctor-patient relationship tends to be optimistic.There are individual differences,media opinions and macro social environment,and economic factors and hospital management are the difficult points to construct harmonious relationship between doctors and patients.The government should continuously enhance the comprehensive line feedback,hierarchical diagnosis and more practice operability,increased health care reform policy propaganda,strengthen grassroots medical institutions construction and supervision,strictly control the hospital expansion budget and decentralization of hospital personnel rights,promote common drug bidding and hospital self tender two line coexist;the construction of new media official communication to guide the patients’ cognitive platform.The hospital should strengthen the internal management,QCC,according to JCI accreditation standards for hospital management,the use of medical reports to strengthen risk management and crisis management,the construction of human resources management,promote humanities hospital and learning hospital,developing information technology.
