Efficacy Of Sanhuanglingzhu Decoction In Treating Obese T2DM Damp-heat Trapped Speen Syndrome And Its Effect On Serum RBP4

Purpose:To observe the clinical effect of Sanhuanglingzhu Decoction in the treatment of obesity type 2 diabetes mellitus(T2DM)with damp-heat trapped spleen syndrome and effect the level of retinol binding protein 4(RBP4),then explore its possible mechanism of action.Methods:A total of 60 obese T2DM patients were divided into treatment group and control group according to randomized controlled grouping.with a total of 30 patients in each group.The control group was treated with basic therapy and conventional western medicine.The treatment group was treated with Sanhuanglingzhu Decoction on the basis of the control group.After 12 weeks of treatment,observe the changes of TCM syndromes,body weight,waist circumference,BMI.,FBG,2hPG,HbAlc,FINS,HOMA-IR,blood lipid and retinol binding protein 4(RBP4);and check the blood,urine and fecal routine,liver and kidney function and other safety indicators.All data were analyzed by SPSS 22.0.Result:1.TCM syndrome efficacy:the total effective rate was 89.7%in the treatment group,and the control group was 27.6%,comparing the two groups,P<0.01,the difference was statistically significant.2.TCM syndrome points:After treatment,the total scores of TCM syndromes were reduced in both groups,comparison between groups,P<0.01 comparing with before treatment,P<0.01,the differences were statistically significant.Comparison of individual symptom scores after treatment,P<0.01,the effect of the treatment group was better than that of the control group.3.FBG、2hPG and HbA1c levels:After treatment,the FBG,2hPG and HbA1c in both groups were reduced,comparison between groups in HbA1c and 2hPG,P<0.01,comparison between groups in FBG,P<0.05;comparing with before treatment,P<0.01,the differences were statistically significant.The treatment group reduced FBG,2hPG and HbAlc better than the control group.4.Obesity related indicators:After treatment,the body weighty waist circumference and BMI were reduced in both groups,comparison between groups in BMI,P<0.01,comparison between groups in body weight and waist circumference,P<0.05;comparing with before treatment,P<0.01,the differences were statistically significant.The treatment group reduced body weight、waist circumference and BMI better than the control group.5.FINS and HOMA-IR:After treatment,FINS and HOMA-IR were reduced in both groups,comparison between groups,P<0.05;comparing with before treatment,P<0.01,the differences were statistically significant.the treatment group reduced FINS,HOMA-IR were superior to the control group.6.blood lipids level:After treatment,the levels of TG、TC and LDL-C were decreased,and the level of HDL-C was increased in both groups,comparison between groups,P<0.05;comparing with the before treatment,P<0.01,the differences were statistically significant.the treatment group reduced TG、TC、LDL-C and raise HDL-C were superior to the control group.7.RBP4 level:After treatment,RBP4 were decreased in both groups,comparison between groups,P<0.05;comparing with the before treatment,the treatment group P<0.01,the control group P<0.05.the treatment group reduced RBP4 was superior to the control group.8.Safety:Two groups of patients before and after treatment,the blood、urine and fecal routine,liver and kidney function were not significant changes.Conclusion:1.Sanhuanglingzhu Decoction can improve the TCM syndromes of patients with obese T2DM damp-heat trapped spleen.2.Sanhuanglingzhu Decoction can reduce the levels of FBG,2hPG and HbAlc of patients with obese T2DM damp-heat trapped spleen.3.Sanhuanglingzhu Decoction can improve body weight,BMI and waist circumference of patients with obese T2DM damp-heat trapped spleen.4.Sanhuanglingzhu Decoction can reduce the level of FINS and improve insulin resistance of patients with obese T2DM damp-heat trapped spleen.5.Sanhuanglingzhu Decoction can improve regulate blood lipid of patients with obese T2DM damp-heat trapped spleen.Down TG,TC,LDL-C levels and increase HDL-C level.6.Sanhuangling Decoction can reduce the level of serum RBP4 of patients with obese T2DM damp-heat trapped spleen,which may be one of the mechanisms of its improvement in insulin resistance.
