ObjectiveBased on the survey and research about the amount,distribution,development trend of health manpower,material resources,as well as the utilization efficiency in Henan Province in the year of 2011-2015,find the problem about the allocation of the health resources during the period of 12 th Five-Year in Henan.The Gini coefficient and Lorenz curve were used to analyze the equity of health resources according to the distribution of population and geographical.the main causes of unfair was estimated by with the decomposability of Theil index.A profound analysis of the reasons of unfairness and reasonable suggestions will be made for the government and related departments develop regional health planning,and to provide reference for the investment,so as to promote the health resource allocation more reasonable optimization,to promote the healthy development of Central Plains.MethodsUsing literature research,descriptive research and quantitative research methods to analyze the status of health resources in Henan Province,Using the Lorenz curve and the Gini coefficient to Analysis the fairness of health human resources,and health material resources about geographic distribution and population distribution in Henan Province from 2011 to 2015.Using Theil index to calculate the contribution rate of Differences within region and the difference between region.Results1.The basic information of the health resources allocation in Henan(1)human resources for health:The health human resources in Henan province showed a rising trend in 2011-2015.Among them,the number of health personnel increased from 622445 in 2011 to 771319 in 2015,an increase of the percentage of 19.30;The number of health technical personnel increased from 395232 in 2011 to 519638 in 2015,an increase of 23.94 percentage points;The number of practicing(Assistant)physicians per thousand population increased from 1.66 in 2011 to about 2.10 in 2015;The number of registered nurses per thousand population increased from 1.46 in 2011 to 2.17 in 2015.But still below the national average.(2)health and material resources:health institutions in the decline of total,the number of health institutions declined from76201 in 2011 to 71397 in 2015.The number of hospitals and community health service centers(stations)showed an upward trend year by year in 5 years,The number of township health centers,clinics and village clinics decreased,especially the number of village clinics decreased significantly.There were 489621 beds,which mean 5.16 beds per thousand population.(3)utilization efficiency of health resources:In recent five years,the number of patients and the number of discharged patients showed a rising trend.The average number of physician visits per day rose from 5.6 in 2011 to 6.2 in 2015,but lower than the national average.The average daily hospital stay in 2015 rose to 2.8,higher than the national average.In the utilization of beds,the utilization rate of hospital beds and the average daily working hours are much higher than that of the hospitals.2.The fairness of health resources allocation in Henan(1)Analysis of equity of health resources allocation by Gini coefficientThe Gini coefficients by population distribution:In 2015,the Gini coefficient of manpower and material resources in Henan province was below 0.2,The Lorenz curve is small,close to the absolute average.It shows that the health resources are in the best fair condition according to the fairness of population distribution.Among them,the bed Lorenz curve is close to the absolute average,which indicates that the bed allocation is the best.The Gini coefficients by geographical distribution:In addition to the Gini coefficient of the institutions is between in the 0.2-0.3.Beds,practicing assistant physicians,registered nurses are between 0.3-0.4.In particular,the Gini coefficient of nurses is very close to the 0.4 warning line.The Lorenz curve has a large degree of curvature,it shows that the distribution of health resources is poor.(2)Analysis of equity of health resources allocation by Theil indexThe total Theil index of Beds and physician is relatively small,but the nurse is the highest.It illustrates that the province has the worst allocation of nurses.Through the decomposition of Theil index,Institutional imbalance is mainly due to differences between regions.contribution rate of beds,doctors and nurses was greater than intra-regional,It is indicated that the imbalance of beds,doctors and nurses is mainly due to the interregional differences.Conclusions1.The health human resources in Henan province is increasing,but the total amount is still insufficient,and the distribution is not balanced.The allocation of nursing staff is the worst,The ratio of doctor and nurse is much lower than the national standard.2.The health resources of Primary medical institutions is insufficient,and the utilization efficiency is low.3.The equity of health resources allocation in Henan is in a state of continuous improvement.The equity of population is better than the regional equity.Regional differences are the main cause of the overall difference.
