Observation Of The Therapeutic Effects Of Selective Early Glottic Carcinoma Treated By SCPL Or Laryngomicrosurgery

Objective:We compare the efficacy and other prognostic differences between SCPL and Laryngomicrosurgery in the treatment of some early glottic cancer with indications,and to provide evidence for the treatment of such patients.Methods:We collected 47 cases of glottic carcinoma admitted to our hospital from January 2007 to December 2011,who were confirmed by pathology and accepted the supracricoid partial laryngectomy(23 patients,group B)or Laryngomicrosurgery(24patients,group A).we searched their medical records and followed them up(on phone or outpatient services).First of all,we used chi square test or nonparametric rank sum test to univariate analysis of the independent factors of postoperative recovery,such as gender 、 age.Then we separated them by the 2 different forms of treatments,Comparative analysis was given to the 2 groups(group A and group B).Results:We succeeded getting 43patients’ follow-up data,22 from group A,while 21 from group B.The follow-up time of group A ranged from37-119 months(Average follow-up time:90.2months),group B ranged from 27-116months(Average follow-up time:79.8months).speaking of the 5-year survival rate,both groups had a enjoyable outcomes:group A90.9%,group B 90.5%.For the very persons we studied,no independent factor was found to influence the postoperative recovery,but we found that if the carcinoma reappeared,it would extremely reduced the patient survival time.In this research,5patients had relapses in 5 years after surgeries(all were local relapses),while 4 of whom died.When we used nonparametric rank sum test to analyse the hospitallization time and expense;t test to analyse the surgical time compareatively,we reached a conclusion that the Laryngomicrosurgery could substantially reduce the time and money.P<0.05 has statistical significance.Conclusions:Both supracricoid partial laryngectomy and Laryngom-icrosurgery show the satisfactory outcomes of oncotherapy,the 2 selections maintain major functions of throat,to name few:breath、speak、swallow.Moreover the latter one totally avoids tracheotomy and incision in the neck,shortens surgical and hospitalized time.The patients who received Laryngomicrosurgery recovered much faster and kept living without aspiration as well as spent less money in the hospital.To reach a conclusion,Laryngomicrosurgery is a advantageous therapy for the early glottic carcinoma if we select the very proper patients.
