Objective: To explore the clinical effect of traditional vertical laryngectomy andimproved vertical partial laryngectomy in the treatment of laryngeal carcinoma.Methods: Retrospectively analyzed on the results and prognosis in patientsunderwent traditional vertical laryngectomy and improved vertical partiallaryngectomy between2003and2009,compare two operation methods on the patientswith laryngeal function and the differences of survival.Traditional group consisted of34patients,classified accordingly by UICC standard of years2002;5cages ofT1bN0M0,26cages of T2N0M0,1cage of T2N1M0,2cages of T3N0M0, the improvedgroup consisted of27cases3cages of T1bN0M0,18cages of T2N0M0,3cages ofT2N1M0,3cages of T3N0M0.Results: Pronunciation function:In the traditional group6cases of normal,fluent in11,moderate in3,and poor in1case, In the improved group16cases of normal,fluent in6,moderate in1.Swallowing function:In the traditional group9cases noaspiration,10cases mild aspiration,3cases moderate aspiration;In the improved group20cases of no aspiration,2cases mild aspiration,3cases moderate aspiration.Respiratory function: In the traditional group33cases had the decannulation,1caseshadn’t the decannulation,the rate of decannulation was91.1%(31/34);In theimproved surgical group27cases had the decannulation,the rate of decannulation was100.0%(27/27). Survival rates:The three-year calculate survival rate was91.7%inthe traditional group and94.5%in the improved group,the five-year calculate survivalrate was86.3%in the traditional group and87.8%in the improved group,by thestatistics,two sets of data analysis without statistical significance.Conclusion: The survival rate was no difference between the two groups,but inlaryngeal functional preservation improved group is superior to the traditionaloperation.
