The Correlation Analysis Of Serum TIgE And EO% In Peripheral With Eos CRSwNP And Neu CRSwNP

Objective: Through the study of patients with nasal polyps in nasal polyps tissue eosinophil infiltration by neutrophils and type in chronic rhinosinusitis,analyze the relationship between the eosinophil percentage of cells and immune pathological acid and total serum Ig E,peripheral blood contrast,To explore a more effective and practical type of immune pathology for CRSw NP:eosinophilic nasal sinusitis with nasal polyps(Eos CRSw NP);neutrophilic nasal sinusitis with nasal polyps(Neu CRSw NP).In order to guide the development of more personalized and accurate CRSw NP perioperative treatment program.Methods: In the selection of 2014-8-1~2016-10-31 underwent functional endoscopic sinus surgery,in Department of ENT in the Second Affiliated Hospital of Dalian Medical University.All the 57 patients are diagnosed as chronic rhinosinusitis with nasal polyp(CRSw NP)according to clinical symptoms,signs,sinus CT and postoperative pathology.The Age is from 18 to 78,with an average age of 48.4 years.The research of this experiment was approved by the ethics committee,and all patients signed informed consent.The tissue in vitro fixed by 10% neutral formalin.The samples were fixed in 4% paraformaldehyde at room temperature for 24 hours and then embedded in paraffin.Continuous paraffin block,the thickness is about 3 um,and dewaxing,hydration.Finally,hematoxylin eosin staining(HE staining)stains tissue.Combined with the immune pathological analysis,CRSw NP will be divided into Eos CRSw NP and Neu CRSw NP 2 types.Eos CRSw NP accounts for 34 cases,including 23 male cases and 11 female cases,Neu CRSw NP accounts for 34 cases,including 18 male cases and 11 female cases.Using the SYSMEX XE-2100 D automatic blood analyzer,Allergy Screen allergen detection system and Beijing perlong enzyme acquisite the data of serum tIgE and EO% among the 57 patients,Among them,when serum tIgE is higher than 100IU/ml,it is defined as the positive rate of tIgE expression.According to the different concentration value it is divided into 3 levels,first level represents the content of normal,tIgE detection value is less than 100IU/ml,higher grade II representative content,tIgE detection value in 100IU/ml-200IU/ml,on behalf of class III is significantly increased,tIgE detection value greater than 200IU/ml.Use SPSS22.0 statistical software for statistical analysis and comparison of the data,and Use t test,X~2 test,Mann-Whitney test,and P < 0.05 said the difference was statistically significant.Explore the relationship between the serum tIgE and EO% in Eos CRSw NP and Neu CRSw NP.Results: 1.CRSw NP was divided into Eos CRSw NP and Neu CRSw NP two groups according to the inflammatory cell counts in nasal mucosa.There were 34 cases of Eos CRSw NP,Neu CRSw NP had a total of 23 cases.2.There are statistically significant differences in the distribution of positive expressionrate in serum immunoglobulin tIgE in Eos CRSw NP and in Neu CRSw NP(X~2=5.070,P=0.024),the positive expressionrate in serum immunoglobulin tIgE of Eos CRSw NP is higher than that of Neu CRSw NP.In Eos CRSw NP,tIgE is mainly concentrated in the third grade(58.8%),in Neu CRSw NP,tIgE is mainly concentrated in the grade II(56.5%),the difference is statistically significant(Mann-Whitney Z=-2.680,P=0.007).3.The distribution of EO% in peripheral blood in Eos CRSw NP and Neu CRSw NP is statistically significant(t=9.035,P=0.004),Eo% in Eos CRSw NP is higher.Conclusion: 1.The expression of tIgE in serum and EO% in peripheral blood was correlated with the infiltration of eosinophils and the infiltration of neutrophils in nasal polyps.2.According to the patients with nasal polyps in eosinophil and neutrophil infiltration,CRSw NP will further more the immune pathological type practical: Eos CRSw NP and in Neu CRSw NP.3.The pathological typing of CRSw NP can guide the formulation of individualized perioperative treatment.For the infiltration of eosinophils in the main,we can use allergic therapy;for the infiltration of neutrophils,antibiotics can be used for anti infection treatment.
