ObjectiveThe paper aims to investigate the surgical complications by posterior spinal orthopedic surgery.methodsWe retrospectively analyzed the data of 199 patients with scoliosis treated in the First Affiliated Hospital of Zhengzhou University from January 2010 to February 2016.All patients by posterior spinal orthopedic surgery without neurophysiological monitoring. There were 71 males and 128 females, with an average age of 15.4 years(aged range from 10 —43)and the CS patients were 61,the AIS patients were 138.According to the different age, gender, etiology classification, different Cobb angle(main bend),different BMI were grouped. We analyzed the incidence of various types of complications. We assessed the health-related quality of life(HRQL)before and after surgery using the Scoliosis Research Society(SRS)SRS-22 score and record the operation time and blood loss.We measure the angle of scoliosis before surgery and at the last follow-up.ResultsThe mean follow-up was 4 years(2.5-6.5)for all patients. The average operation time was 258 min,and the average amount of bleeding was 1028 ml.The average preoperative Cobb angle was 65.78°±28.37° degrees, and the average Cobb angle was 24.35°±7.58° at the last follow-up.The correct rate was about 62.98%.The average preoperative SRS-22 score was63.78±14.86,with the last follow-up of 82.63±6.86.There were 26(13.06%)cases with complications in 106 patient safter one-stage posterior spinal orthopedic surgery without neurophysiological monitoring. The complication of incisional drainage, infection, cerebrospinal fluid leakage, neurological complications and revision for all patients was 13(6.53%),3(1.5%),1(0.5%),4(2.01),5(2.51%).The incidence of neurological complications in CS group was 6.56%,higher than that of AIS group(P ≤0.05),the difference between the two groups was statistically significant. The overall complication rate of congenital scoliosis(CS)was 21.31%,which was higher than that of idiopathic scoliosis(AIS).The difference between the two groups was statistically significant. There was no significant difference in the incidence of other complications.BMI < 18.5 group of patients, the complication of incisional drainage rate is 9.65%,higher than the BMI = 18.5 of 2.35% patients(P < 0.05),there is significant difference between the two group. There was no significant difference in the incidence of other complications.Cobb angle(main bending)≥90 degrees in patients with neurological complications incidence rate was 7.01%,higher than the Cobb angle< 90 degrees group(P < 0.05),there was no significant difference in the incidence of other complications. There was no significant difference in the incidence of complications in patients with different gender and ageConclusionThe effect of posterior surgical treatment of scoliosis is good, and has a high degree of patient satisfaction. The incidence of neurological complications of CS was higher than that in group AIS,the incidence of complications in CS group was higher than that in group AIS.Orthopedic surgeons should pay more attention in treating of congenital scoliosis and patients who were malnourished.
