Objective To investigate the clinical value of low-dose of polyethylene glycol(Shu Taiqing)combined with castor oil or Senna on cleansing in the intestine tract.Method186 hospitalized patients treated in our hospital between between August,2015 and December,2015 were selected and randomly divided into group A[62 cases administered with standard dose of polyethylene glycol(Shu Taiqing)],group B[62 cases administered with low dose of polyethylene glycol(Shu Taiqing)combined with castor oil.],and group C[62 cases administered with low dose of polyethylene glycol(Shu Taiqing)combined with Senna].Ottawa scale and score table for the intestinal bubble were used to evaluate the effectiveness of intestinal cleansing and the bubbles condition. The electrolytes, the ratio of cocci to bacillus and the p H values in feces were monitored before and after the intestinal cleansing. In addition, the adverse reactions, tolerance, compliance and willingness to use again were evaluated by using questionnaires.Results6 cases in total(2 cases in each group)were excluded because they have not completed the colonoscopy.Eventually,180 cases(60 patients in each group)are included in the final analysis. The results are comparable because there are no significant statistical differences in the basic information between the three groups(P>0.05).The detailed results are as follows.1 The Effectiveness of Intestinal Cleansing1.1 Ottawa scale was used to evaluate the effectiveness of intestinal cleansing. There were no significant statistical differences in the score of each intestinal segment, the total fluids, and the overall score by the non-parametric tests(P all >0.05)between the three groups;1.2 There were no significant statistical differences in the effectiveness of intestinal cleansing in each intestinal segment and overall high quality intestine tract between the three groups(P all >0.05);1.3 No significant statistical difference was found in the qualified rate of intestinal preparation between the three groups(x~2=0.960,P=0.619,P>0.05).2 The Intestinal BubbleThere were significant statistical differences in the elimination of intestinal bubbles between the three groups(x~2=11.968,P=0.003,P<0.05).Significant statistical differences were found in the group A compared with group B(c 2=8.292,P=0.004,P<0.05),and group C(c 2=6.536,P=0.011,P<0.05).In contrast, there is no significant statistical difference between group B and C(x~2=0.152,P=0.697,P>0.05).3 Changes of the Electrolytes before and after Intestinal CleansingPaired sample t test was used to analyze the results of the electrolytes before and after intestinal cleansing of the three groups. And the P value was less than 0.05 and significant statistical differences were found between the three groups. But all the P values were greater than 0.05 when the changes of the electrolytes were analyzed by using one-way analysis of variance, indicating that there were no significant statistical differences between the three groups.None of the carbon dioxide combining power after intestinal cleansing in the three groups were less than the normal lower limit. There were no significant statistical differences in carbon dioxide combining power after intestinal cleansing between the three groups(P=1,P>0.05).In addition, all the P values were greater than 0.05 when the number of patients with the concentration of potassium, sodium, chlorine and calcium below the normal lower limit were compared with the total number of the patients in the three groups, indicating no significant difference between the three groups.4 Changes in the Ratio of Cocci to Bacillus and the PH Values in Feces before and after the Intestinal CleansingNo significant statistical differences were found in the ratio of cocci to bacillus and the PH values in feces before and after the intestinal cleansing between the three groups(P all >0.05).5 Adverse ReactionsThere were significant statistical differences in the adverse reactions between the three groups(x~2=10.673,P =0.005,P<0.05).Significant statistical differences were found in group A compared with group B(x~2=7.566,P=0.006,P<0.05),and with group C(x~2=6.0,P=0.014,P<0.05).But there were no significant statistical differences between group B and group C(x~2=0.12,P=0.729,P>0.05).And there were no statistically difference in nausea, vomiting, abdominal pain, abdominal distention were compared separately between the three groups(P all >0.05).6 Tolerance,Compliance and Willingness to use againThere were significant statistical difference in the patient’s tolerance to the liquid for intestinal cleansing between the three groups(c 2=20.902,P=0.002,P<0.05).Significant statistical differences were found between group A and group B(c 2=13.255,P=0.004,P<0.05),and between group A and group C(x~2=10.904,P=0.012,P<0.05).But there was no significant statistical difference between group B and group C(x~2=0.342,P=0.843,P>0.05).No significant statistical difference was detected in the compliance between group A,group B and group C(x~2=4.045,P=0.132,P>0.05).There were significant statistical difference in the willingness to use again between the three groups(c 2=10.673,P=0.005,P<0.05).And significant statistical difference was detected in the willingness to use again between group A and group B(x~2=7.566,P=0.006,P<0.05),and between group A and group C(x~2=6.000,P=0.014,P<0.05).But no significant statistical difference was found between group B and group C(x~2=0.120,P=0.729,P>0.05).7 Costs of Intestinal Cleansing and Economic ValueThe total cost of group A is 154.8 yuan, while group B is 85.4 yuan, and group C is88.4 yuan;Therefore,low-dose polyethylene glycol combined with castor oil or Senna for intestinal preparation is more economical, which can reduce the burden of the patients.Conclusion1.The effectiveness of intestinal cleansing by using low-dose of Polyethylene glycol combined with castor oil or Senna are similar to that by using the standard dose of Polyethylene glycol. The similar effects were observed in the changes of the electrolytes and intestinal microecology(the ratio of cocci to bacillus and the PH values in feces).Low-dose of Polyethylene glycol combined with castor oil or Senna has obvious effects on the elimination of intestinal bubbles, less adverse reactions and tolerance, significantly increased willingness to use again, which reduces the financial burden of the patients.Therefore,low-dose of Polyethylene glycol combined with castor oil or Senna for intestinal cleansing is worthy to be promoted.2.The sample size is small. The larger sample size is required to further observe the effects of low dose of Polyethylene glycol combined with castor oil or Senna on the electrolytes and intestinal microecology in patients(the ratio of cocci to bacillus and the PH values in feces).
