| Objective:To examine the significance of SMS supervision on the management of tuberculosis(TB)patients in improving the treatment compliance and success rate, and to provide scientific evidence for the promotion of the new model and formulation of related polices and measures. Methods:Six counties(districts)were selected by stratified cluster sampling method, and randomly divided into control group and intervention group.TB patients eligible to the condition were included in the study.Mobile message on the management and regular education of core knowledge about TB were carried out on the patients in intervention group. The traditional management model was carried out in control group.Data was collected by questionnaire method.Epi Data and SPSS software were used for data entry and statistical analysis. Results A total of 350 patients were included in the study,160 cases in the intervention group and 190 cases in the control group. There were 270 males(77.1%)and 80 females(22.9%),the average age was 49.11±15.58 years. There was no significant difference in gender, occupation and education level the two groups, therefore, the two groups were comparable. The treatment completion rate in intervention group(96.25%)was significantly higher than that in the control group(86.84%)(χ2=9.52,P=0.002).The visit rate(χ2=7.55,P=0.006)and frequency of visit(t =3.93,P<0.001)in intervention group were significantly higher than that incontrol group. The cost of treatment in the intervention group were significantly lower than that in the control group(t =-6.87,P<0.001).97.5% of the patients would choose to continue taking if symptoms disappear after a period of treatment in the intervention group,however,86.32% of the patients would choose to continue taking in the control group, and the difference was significant(χ2=13.86,P<0.001).The treatment interruption rate and drug leakage rate in the intervention group were significantly lower than that in the control group(χ2=10.41,P=0.001).There was significant difference in drug leakage rate between the two groups(χ2=28.54,P<0.001).Significant difference was also exist in receiving healthcare advocacy between the two groups(χ2=22.60,P<0.001).The review rate of sputum at the end of 2 months and 6months and review rate of X-ray examination in the intervention group were significantly higher than that in the control group. There were significant difference in awareness of TB severity,Transmission, prevention pathway,TB suspects, preventive effect of the vaccine, discrimination against TB patients and affect work between the intervention group and control group. There were significant difference in awareness of TB severity,Transmission, prevention pathway,TB suspects, professional bodies to treatment,TB can be cured, free national policy, taking time, preventive effect of the vaccine, discrimination against TB patients and affect work between pre-treatment and post-treatment among intervention group. Conclusions SMS supervision management model can effectively improve the treatment completion rates, the rate of visits, treatment compliance and review rate.SMS supervision management mode can reduce the rate of treatment interruption in patients, the rate of drug leakage, and reduce health care costs of patients. The model can effectively improve patient awareness of TB-related knowledge, and enable patients to better realize severity of TB and the importance of the treatment rules, and it was important for treatment compliance and social stability and unity. The new modehelps to solve the lack of the traditional model, and it was very important to improve treatment success rate. Therefore, mobile message on the management of TB patients should be promoted. |
