Background:Pancreatic cancer is one of common malignant gastrointestinal tumors whose morbidity and mortality rates are increasing year by year. At present, surgery is the only way to cure pancreatic cancer completely. However, due to its biological and anatomical features, the early detection rate of pancreatic cancer is very low. With high risk of relapse and metastasis after surgery and insensitive to conventional therapies such as diotherapy and chemotherapy, the development of treating pancreatic cancer is still in stasis, which brings lots of suffering to patients. At the same time, more and more literatures reveals that traditional Chinese medicine has its unique advantages in relieving sufferings, improving life quality and reducing harmful side effects of diotherapy and chemotherapy. Chinese medicine is gradually becoming an effective way of treating pancreatic cancer.Objective:This clinical observation is based on the basic theory of TCM and applies the method of warming Yang and dispersing blood stasis gemcitabine into treating middle and advanced pancreatic cancer. It also assesses clinical efficacy and safety of treating the middle and pancreatic cancer with integrated Chinese and western medicine, providing reference for clinical treatment.Methods:48 patients with middle and advanced pancreatic cancer belonging to blood stasis due to Yang deficiency were grouped into experimental group and control group at random, with 24 cases each.Patients in experimental group were treated with gemcitabine and modified prescription of warming and activing yang to remove blood stasis (prepared Monkshood slices lOg, Ginseng 30g, Astragalus mongholicus 30g, Cassia twig 10g, Rhizoma zingiberis 5g, Angelica sinensis 10g, Colla Cornus Cervi 15g, Pericarpium citri reticulatae 5g, Herba Ephedrae 3g, Hirudo 5g, Tabanus 5g, Ground Beeltle 5g,Peach kernel 10g, Flowers carthami 10g, Ligusticum wallichii 10g, Liquorice 3g) from the day before chemotherapy to the end of the second round of chemotherapy. This prescription was created by Professor Li Shijie. Patients in the control group were treated with gemcitabine only. The change of TCM syndrome, hematologic toxicity, liver and kidney function, CA199, tumor, KPS score, and weight before and after treatment were observed and recorded to evaluating the efficacy and safety of two project:method of warming and activing Yang to remove blood stasis with Gemcitabine and single gemcitabine.Results:1. The effective rate and CA199 changes in two groups have no statistical significance after treatment(P>0.05);2.The experimental group can significantly improve TCM syndrome, compared with control group (P<0.05);3. The life quality of patients in experimental group obviously better than the control group(P<0.05);4. Weight change of patients in experimental group is better than that of control group, which has statistical signifieance(P<0.05);5.The incidence of liver and kidney function damage, the blood toxicity especially white blood cells and platelets of patients in experimental group is lower than that of control group.Conclusion:The method of treating middle and advanced pancreatic cancer with warming and activing yang to remove blood stasis and gemcitabine is contribute to relieve suffering, improve quality of life, increase patients’ weight and reduce the adverse reaction caused by chemotherapy.
