Clinical Observation Of Xiaoxianxiong Decoction Combined With Buyanghuanwu Decoction In Treatment Of Early Diabetic Nephropathy

Objectives: Western medicine treatment of early diabetic nephropathy in addition to controlling blood sugar, blood pressure, to correct dyslipidemia, proteinuria no special control method, Chinese medicine is relatively safe, less adverse reaction, widespread clinical application. Along with social progress, improved living conditions, combined with modern clinical findings in patients with type 2 diabetes, about 80% of patients with obesity, dyslipidemia and more. The traditional "Yin hot" theory of prescription medication, and can not effectively improve the condition. To observe Xiaoxianxiong Decoction Combined with Buyanghuanwu Decoction clinical efficacy of early treatment of diabetic nephropathy, to provide a theoretical basis for clinical application.Materials and methods: 86 patients were randomly divided into treatment group and control group. Both groups were control diet, diabetes education, reasonable exercise to control blood sugar(insulin aspart 30 or oral administration of metformin daily 0.25g-0.5g three times a day, so that patients fasting plasma glucose <7.0 mmol/L, 2-hour postprandial blood glucose <11.1 mmol/L), control of blood pressure(40mg telmisartan tablets once a day, so that blood pressure <130/80 mm Hg, poor blood pressure may increase with other antihypertensive drugs) and other basic treatment. The treatment group Xiaoxianxiong Decoction Buyanghuanwu soup.Both groups were treated for three months.Clinical efficacy, symptom score(dry mouth and throat, abdominal pain, weakness weak waist, heat, fatigue, dry stools,tongue yellow), and detection before and after treatment in urine-N- acetyl-β-D-glucosaminidase(NAG), β2-microglobulin(β2MG), urinary albumin excretion rate(UAER), serum creatinine(SCr), blood urea nitrogen(BUN), fasting blood glucose(FBG), total cholesterol(TC), triglyceride(TG), low density lipoprotein(LDL) and high density lipoprotein(HDL).All patients were treated for 3 months. The changes of the therapeutic efficacy, Chinese medicine syndrome scores,NAG, β2MG,urinary albumin excretion rate(UAER), serum creatinine(SCr), blood urea nitrogen(BUN), fasting blood glucose(FBG), total cholesterol(TC), glycerol triester(TG),low-density lipoprotein(LDL),high-density lipoprotein(HDL).Results: 1、The total effective rate in treatment group was 87.8%,which was obviously higher than that in control group(62.7%), P<0.05.2、Compare the efficacy of two symptom improvement rates were significantly different, P<0.05, P<0.01.3、Levels of NAG、β2MG after treatment in treatment groupdecreased more significantly than before treatment,P<0.05, P<0.01.The control group after treatment has improved, but the difference was not significant(P>0.05)4、Levels of UAER、SCr、BUN after treatment in treatment groupdecreased more significantly than before treatment,P<0.05, P<0.01.5、Levels of FBG、TC、TG、LDL after treatment in treatment groupdecreased more significantly than before treatment,P<0.05, P<0.01.After treatment in treatment group,levels of HDL increased more significantly than before treatment, P<0.05.Conclusion: 1、Xiaoxianxiong Decoction Combined with Buyanghuanwu Decoction could improve symptoms,such as dry mouth,bloating,weakness,heat, constipation.2、Xiaoxianxiong Decoction Combined with Buyanghuanwu Decoction could decrease NAG、β2MG in urine,which prompt renal tubular function could improved.3、Xiaoxianxiong Decoction Combined with Buyanghuanwu Decoction could decrease UAER、Scr、BUN,which prompt glomerular function could improved.4、Xiaoxianxiong Decoction Combined with Buyanghuanwu Decoction could decrease lipid significantly.
