Research On The Design Strategy Of LDR Delivery Mode Of Obstetric Nursing Unit Under FCMC Nursing Mode

Most hospitals in our country at present are still using the traditional nursing and delivery mode. Pregnant women are frequently transferred at each stage, such as labor, childbirth and the postpartum recovery maternal, increasing risk of maternal transfer, not easy for family members accompanying, the whole process is also not humane. With the improvement of living standards, People's demand for hospitals and evaluation standards are also improving. Medical industry is gradually transferring from biomedical model to bio-psycho-social medical model. Seeing a doctor means changing from the treatment to a whole range of social services, national policy also actively encourages and promotes the process. For this special group of pregnant women, the family centered medical care model(FCMC) is being increasingly widely accepted. LDR as a kind of good with the FCMC to carry out the core of the mode of delivery, with the times more or less has been introduced into our country. Now the high-end maternity hospitals in the city of our country are learning FCMC from developed countries. A large number of public hospitals appear dole delivery mode, allowing family members accompanying in the side, but because of the subject from the traditional mode and do not reduce the number and distance of transfer, fail to reflect the core of LDR. Private hospitals are fully implemented LDR way, but because of high medical resources sharing, and the high cost of delivery in hospital, can only be confined to a special small range, can not be popularized. Therefore, in the general trend of the traditional way to more advanced LDR gradually change, the change of delivery mode will promote the evolution of the unit of obstetric care unit.The purpose of this paper from the angle of building make FCMC nursing mode of obstetric nursing unit can better meet the requirements of LDR delivery mode, better evenly split delivery of medical resources, the function more perfect, streamline more convenient, maternity wards and more humane and get rid of the shackles of the high price, further suited to China's national conditions, serve the majority of the population. According to the design of the current LDR delivery room and obstetrics nursing unit, this paper puts forward the optimization strategy, and gives some optimization model.This paper starts from a clear distinction FCMC nursing mode, LDR delivery mode and LDRP delivery mode, then summarizes domestic and foreign research results, and then analysis the respective advantages and disadvantages between traditional mode of delivery and LDR/LDRP mode of delivery at home and abroad. The results show that LDR/LDRP mode is superior to the traditional mode of delivery:(1) Reducing the number of maternal transfer in the process of delivery;(2) Shortening the transfer distance from maternity ward to the delivery room;(3) The responsibility system makes the medical resources respond moreefficiently;(4) Family members accompany and family warm decoration make childbirthmore humane.According to the comparison between LDR and LDRP mode, LDR mode is thought to be more appropriate in the present development of China, for individual maternal share medical resources are relatively less, ordinary ward is suit for Chinese labor practices, and the price is relatively lower, therefore this paper carries out the study of the basic knowledge.Then from three aspects of the layout size, the flow control technique and the humanized design, the design composition of LDR delivery room is analyzed in detail. From the stage of functional requirements determine the layout of the room, distance from the human activities and equipment turn to decide the plane size, the shadowless lamp and other equipment installation requirements determine the size of the facade; and analyze and summarize all current typical LDR delivery room in function space, infection control and three aspects of the advantages and disadvantages, and then combined with the optimization strategy of our country economy level and people's labor practices are LDR delivery room and suggests the formation of an optimization based model.Then according to the pre-collection of literature and investigating different cities and scale hospital obstetric care elements, comparative analysis the advantages and disadvantages of the obstetric nursing unit, obstetric nursing unit current LDR method:(1) the western standard LDR mode due to labor habits and economic factors arenot fully adapted to China's national conditions;(2) the domestic varieties of Dole methods can not reflect LDR to reduce thenumber and distance of the main transfer;(3) the traditional way directly into the LDR mode of care units in the roomfunction size and transfer line can not meet the new requirements;(4) the domestic LDR and LDRP delivery mode mixed chaos.(5) currently the LDR mode prices are still too high, LDR can not be popularizedin the domestic public.From the above points, this paper gives the optimal design strategy of obstetric nursing units from three aspects of functional structure, scale size and human nature design:(1) ward and delivery room with the layer and by a number of wards around aLDR delivery room, "LDR group" constitutes a maternity care unit;(2) the nurse in the medical part of the space part of the LDR into the formation ofa group of nursing group";(3) LDR delivery room by the side of the corridor back 600 mm distance, breakthe corridor of a single space;(4) the new human space, such as the activities of the room and air gardens, etc.According to the above several points in the existing typical single corridor nursing unit mode, give the optimization of the basic model.And then combined with the above conclusions, based on the Hanzhong City People's Hospital change extension project in women and children's Hospital of obstetrics nursing unit as the object, applying to the establishment of the LDR delivery and obstetric care unit based optimization model, using the optimization strategy of the plane, confirmed in the service of the same number of personnel on the basis of the transfer times and less distance than the original scheme. The FCMC service mode more reach the designated position, humanized space more, LDR way carrying out more thorough results to verify its feasibility.Finally, summarize the work, put forward the problems and prospects.
