| Objective:we collect postoperative complication,effect,satisfaction and scar size on chest wall of ear reconstruction using big expander and exceedingly expansion method in our department,aim to analyze its advantage and disadvantage,and to investigate its application value in clinical work. Methods:First,we review the 52 microtia cases underwent total ear reconstruction in our department during 2014 Jan and 2014 Dec.The procedure of our method is divided into three stage.In the firs t stage,we made the incision along the remant ear above the lobule,implant the 150 tissue expander subcutaneously in the mastoid region, use the intense pulse light depilation in the late stage of injection and maintain expanding time.In the second stage,we implant the costicartilage framework without skin graft through the primary remnant ear incision.In the third stage, we repair the reconstructed ear appropriately.Meanwhile,the other 37 cases using Zhuang’s ear reconstruction method in our department during 2007 May and 2009 Oct were selected as control group. At last, we contrast the postoperative complication,effect of reconstructed ear,satisfaction and postoperative scar size of the two group. Results:(1)(1)In the experimental group,the percentage of excellent reconstructed ear is 88.5 %.In the experimental group,the percentage is 92.0%.P>0.05,the difference is not statistically significantly.(2)In the experimental group,the percentage of reconstructed ear with asymmetry cranioauricular angle is 71.2%. In the control group,the percentage is 48.7 %.P<0.05,the difference is statistically significantly.(2)In the experimental group, the satisfaction ratio is 91.7%.In the control group,the ratio is 92.6%.P>0.05,the difference is not statistically significantly.(3)(1)In the experimental group,the ratio of expander exposure is 5.8%.In the control group, the ratio is 5.4%.P>0.05,the difference is not statistically significantly.(2)In the experimental group,the ratio of framework exposure is 5.8%.In the control group, the ratio is 5.4 %.P>0.05,the difference is not statistically significantly.(3)In the experimental group,the ratio of framework absorbsion is 3.8%.In the control group, the ratio is 2.7%.P>0.05,the difference is not statistically significantly.(4)The scar size on the chest wall of the experimental group is 1.6±0.5 square centimeter,The scar size group on the chest wall of the experimental is 11.9±2.0 square centimeter,p<0.05,the difference is statistically significant. Conclusioins:Ear reconstruction using intense pulse light depilation,big expander and exceedingly expansion method can supply enough skin to cover the framework,the effect of the reconstructed ear is excellent,the satisfaction ratio is high,the complication ratio is low,and the scar size is small,so it is an effective method. |
