| 【Objective】To investigate the relationship between the ecological environmental elements,the trend of ecological cycle and gastric cancer;To find the relationship between plasma elements and gastric cancer;To research the relationship between the element metabolism related gene polymorphism and susceptibility to gastric cancer;To explore the interaction effects of element metabolism related gene-gene and gene-elements factors.【Methods】1.Ecological study was used to investigate the soil,drinking water,rice,vegetables,pickles and plasm samples.Atomic fluorescence was used to detected Hg,Se,As.ICP-MS was used to detected Ca,Mg,Mo,Mn,Co,Ni,Cu,Zn,Cd,Pb,Fe.Independent samples nonparametric test and ordinal logistic regression were used to analysis the relationship between geochemical elements and gastric cancer and geochemical elements’ recycling in the ecological.2.Case-control study was conducted with 299 patients and 295 control subjects.Patients were collected from the Xianyou county hospital with histologically confirmed.Control subjects were also collected from Xianyou.The control subjects were excluding cancer,digestive tract diseases.To obtain the general situation of the case and control groups,life habits and other informatios from questionnaire survey and blood samples.Blood samples were treated by microwave digestion,and ICP-MS was used to detect the plasma chemical elements.3.Molecular epidemiology method was used in this study.5ml blood samples were collected from every case and control.After the extracting of genomic DNA,the Sequenom platform was usde to determine the genotypes of polymorphisms.Logistic regression was used to analyse the relationship between gene polymorphism and gastric cancer.Generalized multifactor dimensionality reduction,crossover analysis were used to identify the interation between gene-gene and gene-elements risk factors.【Results】1.The results of the trace elements of ecological environment in Xianyou County:(1)1)The soil chemical elements Mg,Cd,Hg,Cu/Zn had statistical differencesamong different mortality of gastric cancer.Pairwise comparisons showed that there were no statistical differences.Ordinal logistic regression showed that gastric cancer was negatively correlated with Mo,Se,while positively related with Pb;The chemical elements of drinking water meet the sanitary standard of drinking water.The drinking water chemical elements Ca,Mg,Mn,Co,Ni and Hg had statistical differences among different mortality of gastric cancer.Pairwise comparisons showed that Ca was increasing with gastric mortality,Mg,Mn,Co in high and middle incidence,high and low incidence have statistical significance,Ni in high and low incidence had statistical significance in the level of area difference.In drinking water,Ca,Mg and Pb were positively correlated with gastric cancer,while Mo was opposite;The rice chemical elements Mg,Mn,Cu,Mo,Pb,Se,Hg and Cu/Zn had significant differences among different mortality of gastric cancer.Mg was positive correlated with gastric cancer,while Mo was negatively related with it;The vegetables geochemistry elements Fe,Mg,Mn,Co,Mo,Ni,Zn,Cd,Pb,Hg and Cu/Zn had statistical differences among different mortality of gastric cancer.It was positively correlated with Fe;The pickles chemical elements Ca、Fe、Mg、Mn、Ni、Cu、Zn、Cd、Pb、Se had significant differences among different mortality of gastric cancer.It was possitive correlated with Mn,while it was negatively correlated with Ca;(2)Element sequence in different ecological changes: it changes from soil to drinking water,from soil to rice.However,it is similar between vegetables and pickles,between water,rice and plasm.2.The relationship between plasma trace elements and gastric cancer:(1)There were differences between case group and control group in their plasma elements.The level of Cu,Mg,Cu/Zn was was higher in case group.However,the level of Mo,Ca was higher in control group.(2)The non-conditional logistics regression analysis showed that gastric cancer was positively correlated with Cu.Mg,however it was negatively correlated with Ca.3.The relevance was found between NKG2D(rs1049174)gene polymorphism and gastric cancer,specially in individuals who carried with rs1049174 GG,rs1049174(GC+GG),OR(95%CI)were 1.85(1.02,3.38),1.50(1.01,2.26).It showed that different haplotypes have different effects on gastric cancer,low risk was showed in NKG2 D gene Ars1746123Trs10431294Grs1049174 haplotype(OR:0.29,95%CI:0.15,0.56),low risk was showed in NKG2 D gene Trs1746123Trs10431294Crs1049174 haplotype(OR:0.33,95%CI:0.22,0.50),high risk was showed in NKG2 D gene Trs1746123Crs10431294Crs1049174 haplotype(OR:,1.81,95%CI:1.40,2.34),high risk was showed in NKG2 D gene Trs1746123Trs10431294Grs1049174haplotype(OR:3.09,95%CI:2.30,4.16).In non cardia cancer,low risk was showed in NKG2 D gene ATrs1746123Trs10431294Crs1049174 haplotype(OR:0.46,95%C I:0.23,0.92).4.Gene-gene,gene-elements interaction analysis:in cardia cancer,rs1332018*r s1695 is the optimal interaction model.In gastric cardia rs1332018*Cu is optim al interaction model.Further analysis shows that there exists positive multiplicati ve interaction between rs1332018 and Cu.【Conclusion】1.The geochemical elements of soil and drinking water in Xianyou county are in line with the requirements of soil environment quality standard and quality standard of drinking water.2.The lack of Mo,Se and excess of Cu/Zn ratio in the body may be the important risk factors for gastric cancer in Xianyou County which may be affected by the external environment;3.Gastric cancer may be influenced by the geochemical elements in different environmental media;4.The pattern of elements distribution in different medium environment are differences.It is consistent between drinking water and the body;5.The genotype GG,(GC+GG)of rs1049174 polymorphism of NKG2 D gene increase the risk of gastric cancer;6.The rs1332018 polymorphism of GSTM3 gene and plasma Cu presence a positive additive interaction in gastric cancer process. |
