Objective:Comparison of surgical effect of the improved method and traditional operation mode on the healing of Achilles tendon rupture inThe animal model of surgical repair of acute Achilles tendon rupture,Methods:72 healthy New Zealand rabbits were divided into 2 groups randomly, each group had 36 rats, and the right hind limb was chosen to be the experimental leg, and the Achilles tendon was cut off at the middle of the Achilles tendon.The control group was treated with pure Kessler method. In the experimental group, the Kessler suture was used to suture the wound, and the absorbable suture was used to fix the border of the tendon and the calcaneus bone. The traction was formed to heal the Achilles tendon in the condition of low tension.In four eight twelve weeks after surgery, choose 12 rabbits sacrificed the Achilles tendon, Achilles tendon length measurement (from the belly and tendon junction to calcaneal insertion),6 of them were used for the determination of ultimate tensile strength, and the other 6 rabbits were examined with histological examination.To compare the length of Achilles tendon, the ultimate tensile force and the results of histological examination.Results:According to the experimental design, the acute Achilles tendon rupture model was constructed, and the Kessler suture method and modified Kessler method were used to repair the acute Achilles tendon rupture.The control group survived 32 rabbits, the experimental group survived 30 rabbits.Four weeks after surgery:the average length of Achilles tendon in the experimental group and the control group were 4.34cm and 4.18cm, respectively, and the mean values were 31.ON and 29.0N;Histological examination:the experimental group of rabbit tendon collagen fibers and arranged regularly; the control group is surrounded by a number of fibroblasts were significantly less, less collagen fibers arranged in disorder.Eight weeks after the operation, the experimental group and the control group the average length of Achilles tendon of rabbits were 5.00cm,5.12cm,123.ON and 88.ON respectively. The mean tension, the difference was statistically significant;Histological examination:the experimental group, the collagen fibers arranged neatly, fibroblast to mature, collagen fibers arranged parallel to the direction; the control group showed further granulation tissue machine, fiber was significantly higher than the experimental group sparse.Twelve weeks after operation, the average value of Achilles tendon length in the experimental group and the control group were 5.08cm and 5.63cm, respectively, and the mean values were 157.4N and 130.2N, the difference was statistically significant;The Achilles tendon was healed in twelve weeks, and the results of histological examination were similar to that at eight weeks.Conclusion:1.Using modified Kessler suture method to repair acute Achilles tendon rupture, eight weeks, twelve weeks Achilles tendon limit tension, histological examination results are better than the common method.2.Modified Kessler suture method based on the simple Kessler suture method to join the internal traction, with reduced tendon healing tension, reduce tendon healing time, promote tendon healing and other advantages.
