Clinical Case Reports Of “All-on-4” Implant Restoration

Objective: The aim of this clinical research is to evaluate the clinical effects of "All-on-4" implant restoration technology of chronic periodontitis patients,to summarize the clinical experiences,to provide the reference of complications' prevention and handling methed.Methods: We selected 5 patients from the implant center in Dalian Stomatology Hospital since 2014.They were people with chronic periodontitis patients,dental arch defection,requiring no teeth absence period and high aesthetic,asking for implant restoration.After peridontal treatment,teeth extraction,"All-on-4" immediate implant immediate restoration was adopted with Nobel Active? implant system.The patients were asked to go for check-up regularly with health maintenance and treatment of complications.When osseointegration were stable and soft tissue was stable,implant-supported fixed partial prosthesis with CAD / CAM cutting titanium bracket and polymer ceramic was taken.After that,regular follow and health maintenance should be kept.Results: Totally 27 implants were inserted,of which 1 implant broken.Totally 6 prosthesis were used.Osseointegration were good,no abnormal marginal bone loss were found in 26 implants.Marginal bone resorption<1.5 mm.4 kinds of complications occurred in temporary prosthesis,including artificial tooth fracture or fall off,prosthesis fracture,screw loose or broken,affect pronunciation.They were handled by a doctor quickly.5 permanent prosthesis were completed.Patients were satisfied with the function and aesthetic effect.Conclusion: "All-on-4" immediate implant immediate restoration,with permanent prosthesis of CAD / CAM cutting titanium bracket and polymer ceramic,is a viable treatment for chronic periodontitis patients after peridontal treatment and teeth extraction.Its long-term effect evaluation requires larger samples and further term clinical observational study.
