| OBJECTIVE: To evaluate the clinical effect of two methods of temporary crown,one is immediately after the implant placement and the other is delayed.Analyze gingival induction on the maxillary aesthetics zone which is edentulous.Explore the feasibility that if the immediate temporary crown,especially the resin neck ring can be used to induce the gingival and what’s the technical details.This study will reduce the tooth-free period,improve implant repair effect,and provide some clinical guidance for aesthetic restoration.Methods: Weifang People’s Hospital Department of Oral Implants collected 32 patients whose anterior teeth of maxillary aesthetic area missed(2014.12-2016.2),the age region is 20-50.The scores of seven major scores of soft tissue esthetic score(PES)are all ≥6 points,exclude serious periodontal disease,severe soft tissue defects,and patients with bruxism.The 32 patients were randomly divided into two groups,each group include 8 males and 8 females.(1)The experimental group(gingival implants were immediately shaped after implantation): The implant was screwed into the temporary abutment immediately after implants to make a temporary crown for gingival induction and permanent crowns after three months;(2)The control group: The finished healed abutment was screwed after the second stage of operation,and the temporary crown with resin neck ring supported by the temporary abutment was worn after 2 weeks.Permanent crowns were made after three months.The aesthetical risk assessment was carried out before dental implants in the both groups.The esthetic restoration effects were summarized,during the process and after the permanent crowns were worn.Satisfaction with the treatment and the aesthetic results also scored.The success rate is calculated after 1 year of permanent restoration.Results:(1)The two groups have no statistically differences in gender,age and PES.(2)the gingival-forming group and the second-stage gingival forming group after implantation had 100% success rate after immediate implantation,had no implant ablation,the intact rate was 100%,and the 16 temporary crowns have no anomalies.(3)The survival rate of the temporary crowns in the control group and the experimental group was 100%,and there are no loss of temporary crown in both groups.(4)The score of the aesthetic evaluation of the experimental group was slightly higher than that of the control group(7.48 ± 0.51 vs.6.85 ± 0.71,P> 0.05);(5)The immediate gingival induction group had 100% satisfactory with dentalaesthetics.Conclusion: The indications patients with temporary crown supported by the implant are conducive to the long-term gingival morphology maintenance.The immediately induction after immediate teeth implants for anterior maxillary single teeth implants aesthetic restoration patients is clinical feasibility,reliable,gingival form are natural,aesthetics is good,patients have high satisfaction.Gingival induction with implant-supported temporary prosthesis can reduce edentulous period,save chair time,and provide good soft tissue aesthetics,so it can replace the traditional method of extension of gingiva induction. |
