Study On The Relationship Between Social Support And Ostomy Adjustment Of Bladder Cancer Patients With Cutaneous Ureterostomy

Objective:To investigate the current status of social support and adjustment in patients with bladder cancer and cutaneous ureterostomy,to identify factors that influence the adjustment,and to explore the relationship between social support and adjustment.The aim of this study is to provide theoretical research formulation and effective nursing care.Methods:A total of 119 patients with bladder cancer and cutaneous ureterostomy were recruited from March in 2015 to March in 2016 by a convenience sampling method from third-grade class-A hospitals in Nanchang.They were investigated with cutaneous ureterostomy patients’ general information questionnaire,social support revalued scale(SSRS)and Chinese version-ostomy adjustment inventory(C-OAI).The data collected was analyzed by SPSS19.0,statistical methods including descriptive analysis,t-test,anova,pearson correlation analysis and multiple linear regression.Results:1.The mean total score of ostomy adjustment was(44.62±8.20).The average score of dimensions from high to low order were persistent concern(2.01±0.60),the active live attitude(2.41±0.48),and acceptance(2.42±0.45).2.The mean total score of the social support was(37.90±5.42),the score of three dimensions were(9.91±3.84),(10.64±2.05),(7.35±1.57).3.The cutaneous ureterostomy of bladder cancer patients’ adjustment scores of different gender,ages,educational level,working condition,residential state,average monthly income,other disease,postoperative time,medical informed consent,ostomy patients association,acceptance of ostomy were statistically different(P<0.05).4.The cutaneous ureterostomy of bladder cancer patients’ social support was significant positively related to their ostomy adjustment(r=0.592,P<0.01).5.Regression analysis showed that factors affecting the adjustment of bladder cancer patients with cutaneous ureterostomy were degree of education,residential state,other disease,postoperative time,acceptance of ostomy,subjective support,support availability.They can explain 72.1% for the variances of ostomy adjustment.Conclusions:1.The cutaneous ureterostomy of bladder cancer patients were in a middle level of ostomy adjustment.The average score of persistent concern dimension was the lowest,the patients’ concern was serious,especially for self-role rebuilding and social activities participation.The average score of acceptance dimension was the highest,and the ostomy was well received.Among all the items,the worst item is the social activities participation.2.The cutaneous ureterostomy of bladder cancer patients were in a middle level of social support.The patients’ social support net was narrow,subjective support and objective support tend to be internal support,and external support was not enough.The patients’ attitude to availability of external support was not active.Among all the items,the worst item is participating in activities.3.The cutaneous ureterostomy of bladder cancer patients’ social support was significant positively related to their ostomy adjustment.More social support,better ostomy adjustment.Social support affect persistent concern greatly,while subjective support affect ostomy adjustment greatly.4.The patients’ level of adjustment was affected by educational level,residential state,other diseases,postoperative time,acceptance of the ostomy,subjective support,support availability.The most important one was acceptance of the ostomy.
